摘要
目的:探讨牙体缺损至龈下残根的正畸修复联合治疗效果。方法:选择龈下牙折患者18例,共20颗患牙,断端最低处距龈缘的距离为1.5~3.2mm。采用多学科综合治疗的方法,对患牙分别进行根管治疗术,正畸牵引术,桩核冠修复等治疗。平均疗程4~6个月。正畸治疗前、后摄X线片,测量残根断缘距牙槽嵴顶的距离,评估正畸牵引效果;牙冠修复后随访1年,评价综合治疗疗效。结果:20颗患牙固位稳定,牙龈健康,美观效果良好。龈下残根经正畸治疗后牙合向平均移动距离3.56mm,修复后患牙冠根比与对侧同名牙相比无明显差异。结论:伤及龈下的残根,只有通过正畸、修复等多学科的联合治疗,才可以满足人们对口腔健康美观日益增高的需求。
Objective:To investigate clinical effect of orthodontic and prosthodontic treatment on sub-gingival residual roots by. Methods: 18 patients with 20 fractured roots were involved and the fracture of the roots extended a-bout 1. 5-3. 2 mm below the gingival margin. An interdisciplinary approach which combining endodontic root canal treatment,orthodontic extrusion,and prosthodontic post-core-crown restoration procedures was adopted. The process of therapy was 4-6 months. The changes of orthodontic treatment were measured by digital dental apical X-rays. Results: The average traction distance of sub-gingival residual roots was 3. 56 mm. The crown restoration remained stable and the gingival tissues were healthy. After prosthetics .the crown root ratio was no significant difference be-tween the dental prosthesis and the contralateral tooth. Conclusion: A multidisciplinary approach to the management of subgingival residual roots is necessary for successful treatment.
