摘要
目的探讨内痔注射加分段多层次弧切缝合术治疗环状混合痔的临床疗效。方法 160例环状混合痔患者按照平行、对照的方法随机分为治疗组(81例)和对照组(79例)。治疗组行内痔注射加分段多层次弧切缝合术治疗,对照组行内痔注射加外剥内扎术治疗,对两组患者临床疗效、手术操作时间、术中出血量、愈合时间、并发症进行比较分析。结果两组临床疗效、手术操作时间、术中出血量比较,差异无统计学意义(P>0.05)。治疗组愈合时间为(6.49±2.12)d,短于对照组的(8.84±3.91)d,差异有统计学意义(P<0.05)。治疗组术后当日视觉模拟评分法(VAS)评分≥7分者2例,尿潴留1例(采用导尿)、水肿0例,优于对照组的9、8、4例,差异有统计学意义(P<0.05)。结论内痔注射加分段多层次弧切缝合术治疗环状混合痔具有术后恢复快及并发症少等优点,可在临床推广应用。
Objective To explore clinical effect of internal hemorroid injection and segmented multilevel arc cutting decompression in treating circumferential mixed hemorrhoid. Methods A total of 160 patients with circumferential mixed hemorrhoid were randomly divided by parallel and control method into treatment group(81 cases) and control group(79 cases). The treatment group received internal hemorroid injection and segmented multi-level arc cutting decompression, and the control group received internal hemorroid injection and external dissection and internal ligation. Comparison and analysis were made on clinical effect, operation time, intraoperative blood loss, healing time and complications in two groups. Results There were no statistically significant difference in clinical effect, operation time and intraoperative blood loss between two groups(P〉0.05). The treatment group had shorter healing time as(6.49±2.12) d than(8.84±3.91) d in the control group, and the difference had statistical significance(P〈0.05). The treatment group had 2 cases of visual analog scale(VAS) score 7 points above on post-operation day, 1 case of uroschesis(treated with catheterization), and 0 case of edema, which were all better than 9, 8, 4 cases in the control group, and differences had statistical significant(P〈0.05). Conclusion Internal hemorroid injection and segmented multi-level arc cutting decompression have advantages of quick recovery and littler complications after operation in treating circumferential mixed hemorrhoid. It is worthclinical promotion and application.
