低剂量CT扫描对COPD及哮喘患者近端气道重塑的对比研究 被引量:23

Comparative Study of Proximal Airway Remodeling in Patients with COPD and Asthma using Low Dose CT Scan

摘要 目的通过低剂量CT扫描对慢性阻塞性肺疾病(COPD)及哮喘患者近端气道定量CT指标进行对比分析,探讨COPD及哮喘患者近端气道重塑情况。方法搜集52例稳定期哮喘患者、69例稳定期COPD患者及20名健康对照人群,行低剂量呼吸双相CT检查,采用多因素分析对三组定量CT气道参数进行统计学分析。结果COPD、哮喘及健康对照组LA/BSA、WA/BSA、TA/BSA、WA%及Pi10WA差异均有统计学意义(P均<0.001),表现为哮喘组LA/BSA及TA/BSA均小于COPD组及健康对照组(P均<0.05),同时哮喘组WA%及Pi10WA均大于COPD组及健康对照组(P均<0.05)。结论哮喘患者近端气道较COPD存在明显的气道重塑,且气道重塑主要表现为支气管管腔面积缩小;COPD患者近端气道重塑并不明显。 Objective To explore the proximal airway remodeling in patients of COPD and asthma,by comparative analysis with quantitative CT airway indicators using low dose CT scan. Methods 52 stable asthmatic patients,69 stable COPD patients and 20 healthy control subjects,with low dose double-phase CT examination were studied using single factor analysis of three groups of quantitative CT airway parameters to evaluate COPD and asthma proximal airway remodeling.Results There were statistically significant difference in the COPD,asthma and healthy control subjects with LA / BSA,WA / BSA and TA / BSA( all P〈 0. 001). the subjects demonstrated as LA / BSA and TA / BSA of asthma were less than COPD and the healthy control subjects( both P 〈0. 05),and WA% and Pi10 WA were greater than COPD and healthy control subjects( both P 〈0. 05). Conclusion The proximal airway remodeling in asthma was significantly more than that in COPD,and airway remodeling was mainly manifested as the reduction of bronchial lumen area. There was no proximal airway remodeling in COPD patients.
