摘要
目的 :探讨诱导型一氧化氮合酶(inducible nitric oxide synthase,iNOS)在乙型肝炎组织中的表达情况及与炎症分级的相关性。方法:收集HBs Ag阳性肝穿刺及手术标本71例,按肝炎修正组织学活性指数(histology activity index,HAI)记分法分为G0、G1、G2、G3、G45组,运用免疫组织化学法检测iNOS在各组中的表达情况,并分析其与炎症分级的相关性。结果:iNOS阳性染色主要定位于肝细胞的细胞浆,部分淋巴细胞胞浆中也可见iNOS阳性染色;随着炎症程度增加,iNOS在肝细胞和淋巴细胞中的表达强度均逐渐增强,且表达强度与炎症分级呈正相关(肝细胞r=0.5148,P=0.0000;淋巴细胞r=0.3491,P=0.0016);肝细胞中iNOS的表达与淋巴细胞iNOS的表达存在正相关关系(r=0.4762,P=0.0000)。结论:iNOS可作为乙型肝炎炎症分级的一项参考指标,可能与乙型肝炎病理生理过程密切相关。
Objective: To investigate the relationship between inducible nitric oxide synthase(iNOS) expression and the histological grading of liver inflammation in hepatitis B patients. Methods:Tissue specimens were collected from 71 hepatitis B patients with HBs Ag-positive. The specimens were divided into group G_0, G_1, G_2, G_3, G_4 according HAI scoring method. Immunohistochemical staining was performed to detect the iNOS expression in all groups. Results:Immunohistochemical results show that the positive iNOS staining mainly locate in hepatocellular cytoplasm, some lymphocytes were also seen in the cytoplasm iNOS positive staining. With the increase of the grade of inflammation, iNOS expression in hepatocytes and lymphocytes strength increased, and the expression intensity and inflammation grading were positively correlated(hepatocytes: r =0.5148, P =0.0000, lymphocytes: r =0.3491,P =0.0016; respectively). There was positive correlation between hepatocellular iNOS expression and T cells(r=0.4762,P=0.0000). Conclusion: Detecting the expression of iNOS is a valuable consult for the histological grading in hepatitis B liver tissues. iNOS most probably played an important role in the pathophysiology process of hepatitis B patients.
