摘要
目的通过分析2011—2015年北京市朝阳区常营社区卫生服务中心门诊患者的资料,为更好地开展社区卫生服务工作提供科学依据。方法采用回顾性分析方法,收集2011—2015年北京市朝阳区常营社区卫生服务中心门诊医生工作站电子系统中的门诊诊疗日志,对近5年的门诊就诊例次、性别、年龄、主要就诊病因、转归类型、就诊类型、医疗费用支付类别等资料进行分析。结果 2011—2015年门诊就诊例次分别为62 428、110 957、136 133、156 062、167 942人次,5年的逐年增长率分别为:77.74%、22.69%、14.64%、7.61%。2011—2015年患者的性别比基本保持稳定,男性约占40.00%;年龄结构也基本保持稳定,60~69岁位居第一,约占总例次的30.00%,其次为50~59岁、30~39岁、70~79岁、40~49岁;第一诊断位列前2位的为急性上呼吸道感染(约占13.00%)和高血压(约占9.00%),其次为支气管炎、骨关节病、2型糖尿病和冠心病,各约占5.00%;转归类型为约98.00%的患者就诊后选择回家;患者的就诊类型以复诊为主,约占90.00%;患者的医疗费用支付以社保卡为主,约占70.00%。结论常营社区卫生服务中心应将社区老年居民作为重点服务人群,将针对上呼吸道感染、高血压、支气管炎、骨关节病、2型糖尿病、冠心病等疾病开展预防和治疗工作作为重点工作,在分配医疗资源和培训医生时对上述方向予以优先考虑。社区卫生服务机构有能力为社区居民提供有效的基本医疗服务并且切实做到持续健康管理,在推行的分级诊疗制度中将发挥重要作用。
Objective To analyze the data of outpatients visiting Changying Community Health Service Center of Chaoyang District of Beijing from 2011 to 2015,so as to provide a scientific basis for the development of community health service.Methods A retrospective review of diagnosis and treatment records was conducted for all patients who visited Beijing Chaoyang District Changying Community Health Service Center from 2011 to 2015.All records were stored in the Outpatient Physician Workstation Electronic System of Changying Community Health Service Center.The data of the patients during the five years,including outpatient visits,gender,age,spectrum of disease,types of outcomes,type of visit,and methods of payment for expense were analyzed.Results The outpatient visited from 2011 to 2015 were 62 428,110 957,136 133,156 062 and167 942,respectively,with year-on-year growth rates of 77.74%,22.69%,14.64%,7.61%.During the five years,the patients' male-to-female ratio( about 40.00%) as well as the patients' age structure were basically stable.The largest proportion of outpatients were in 60-69 years( about 30.00%),followed by 50-59 years,30-39 years,70-79 years and40-49 years.In terms of the prevalence of various diseases,acute upper respiratory infection ranked first( about 13.00%),hypertension ranked second( 9.00%),and four diseases( bronchitis,osteoarthrosis,type 2 diabetes and coronary heart disease) ranked third( each about 5.00%).Among all the patients,about 98.00% went home after the visit with regard to types of outcomes; about 90.00% of them visit for a previously diagnosed problem; approximately 70.00% of them paid the medical expense by social security card.Conclusion For Changying Community Health Service Center,elderly community residents are the key service people,and prevention and management of acute upper respiratory infection,hypertension,bronchitis,osteoarthrosis,type 2 diabetes,coronary heart disease and other common diseases are the major work,and these aspects should be given priority when allocating medical resources and arranging physician training program.Community health service centers can play an important role in the development of hierarchical medical system by offering primary health care and subsequent management for residents.
