摘要
目的研究磺胺嘧啶银联合三黄生肤油对糖尿病大鼠末梢循环障碍及足溃疡的保护作用,并探讨其作用机制。方法选取80只健康Wistar大鼠进行糖尿病造型,将造模成功的70只大鼠随机分为末梢循环障碍组与足溃疡组,每组35只。将造模成功的末梢循环障碍大鼠32只与足溃疡大鼠34只分别随机分为观察组与对照组,2组大鼠均给予磺胺嘧啶涂抹患处,观察组在此基础上给予三黄生肤油涂抹患处,均连续给药35 d。给药期间(1,2,3周)记录末梢循环障碍大鼠足底皮温、疼痛阈值,记录足溃疡大鼠溃疡面愈合情况及外周血血管内皮生长因子(VEGF)水平。结果末梢循环障碍组中,2组给药前2周大鼠足底温度均低于给药前(P均<0.05),且观察组明显高于对照组(P均<0.05);观察组给药3周皮温高于给药前及对照组(P均<0.05),而对照组与给药前比较差异无统计学意义(P>0.05);2组大鼠的痛阈随着给药均逐渐降低(P均<0.05),且观察组给药后各时间点痛阈均明显低于对照组(P均<0.05)。足溃疡组中,观察组给药2周和给药3周溃疡面愈合率均明显高于对照组(P均<0.05);2组给药后各时间点外周血VEGF水平均明显高于给药前(P均<0.05),且观察组给药后各时间点VEGF水平均明显高于对照组(P均<0.05)。结论磺胺嘧啶银联合三黄生肤油能明显改善糖尿病大鼠末梢循环障碍引起的温度及痛觉迟钝,同时能通过提高VEGF的表达促进足溃疡大鼠溃疡面愈合。
Objective It is to study the protection of Silver sulfadiazine combined with Sanhuang-shengfu oil on peripheral circulation disorders and foot ulcer in diabetic rats,and to explore its mechanism. Methods 80 healthy Wistar rats were selected to establish diabetic model,and the successful models( n = 70) were randomly divided into peripheral circulatory disorder group and ulcer group,every group included 35 rats. The successful peripheral circulatory disorders models( n = 32) and foot ulcer models( n = 34) were randomly divided into control group and observation group,both groups were given Silver sulfadiazine on the wound surface,the observation group was added with Sanhuang-shengfu oil on the wound surface. Both groups were treated for 35 days. The plantar temperature,hot pain threshold of rats were detected in the peripheral circulatory disorders group,and the intergrown condition of ulcer,and the level of VEGF in the serum were detected in the ulcer group during treatment( 1,2,3 weeks). Results In peripheral circulatory disorder group,the plantar temperature was lower at 2 weeks before treatment than that before treatment( P〈0. 05),and the temperature in observation group was higher than that in control group( P〈0. 05); the skin temperature at 3 weeks after treatment was higher than that before treatment and in control group( P〈0. 05),but no significant changes was found in control group( P〉0. 05); pain threshold was decreased gradually with treatment time increased in both groups,and the threshold at every time point in observation group were lower than that in control group( P〈0. 05). In ulcer group,the healing rates at 2 and 3 weeks after therapy in observation group were higher than that in control group( P〈0. 05); the levels of serum VEGF at every time point after treatment were higher than that before treatment,and the increase in observation group was more significant than that in control group. Conclusion Silver sulfadiazine combined with Sanhuang-shengfu oil can improve the plantar temperature decrease and pain dysesthesia of rats caused by diabetic peripheral circulatory disorders,and can promote wound healing of diabetic foot ulcers in rats by up-regulation of VEGF.
