布鲁菌感染患者降钙素原、C-反应蛋白、白细胞计数和血沉等炎性指标研究 被引量:6

Study of procalcitonin,C-reactive protein,white blood cell count and erythrocyte sedimentation rate in Brucella infections

摘要 目的对布鲁菌感染患者降钙素原(PCT)、C-反应蛋白(CRP)、白细胞计数(WBC)、血沉(ESR)进行研究。方法将169例血培养阳性患者分为3组,其中布鲁菌组39例、大肠埃希菌组81例、金黄色葡萄球菌组49例,分别对3组PCT、CRP、WBC、ESR的检测结果进行比较。结果 PCT在3组中比较,布鲁菌组均低于大肠埃希菌组和金黄色葡萄球菌组,差异有统计学意义(P<0.05);金黄色葡萄球菌组低于大肠埃希菌组,差异有统计学意义(P<0.05)。WBC在3组中比较,布鲁菌组均低于大肠埃希菌组和金黄色葡萄球菌组,差异有统计学意义(P<0.05);大肠埃希菌组和金黄色葡萄球菌组,差异无统计学意义(P>0.05)。CRP、ESR检测结果比较,差异无统计学意义(P>0.05)。结论血清PCT水平在布鲁菌感染患者中升高不明显,联合CRP、WBC、ESR等指标可以辅助诊断布鲁菌血流感染。 Objective To study procalcitonin( PCT),C- reactive protein( CRP),white blood cell( WBC) and erythrocyte sedimentation rates( ESR) in patients with Brucella infections. Methods 169 patients with positive blood culture results were grouped,including Brucella group with 39 cases,Escherichia coli group with 81 cases and Staphylococcus aureus group with 49 cases. And the detection results of PCT,CRP,WBC,ESR in the three groups were compared. Results The level of procalcitonin in Brucella spp. group was lower than that in Escherichia coli group or Staphylococcus aureus group,with the differences statistically significant( P〈0. 05). The level of procalcitonin in Staphylococcus aureus group was lower than that in Escherichia coli group,with the differences statistically significant( P〈0. 05). The count of WBC in Brucella spp. group was evidently lower than the other two groups,with the differences statistically significant( P〈0. 05). The level of WBC was not statistically significant on the difference between Escherichia coli group and Staphylococcus aureus group( P〉0. 05). There was no statistical significance on the difference among the 3 groups in terms of CRP and ESR( P〉0. 05). Conclusion Procalcitonin did not increase evidently in Brucella spp. group. Combination with C- reactive protein,white blood cell and erythrocyte was useful in the diagnosis of Brucella spp. bloodstream infection.
