高频振荡通气联合肺泡表面活性物质对新生儿ARDS氧合功能的影响 被引量:2

摘要 目的:观察高频振荡通气(high frequency oscillatory ventilation,HFOV)联合肺泡表面活性物质(pulmonary surfactant,PS)对新生儿呼吸窘迫综合征(neonate respiratory distress syndrome,NRDS)氧合功能的影响。方法:选取164例NRDS患儿,随机分为观察组和对照组各82例,观察组给予HFOV+PS治疗,对照组给予常频通气(conventional mechanical ventilation,CMV)+PS治疗,对比两组临床疗效。结果:治疗后观察组动脉血氧分压(partial pressure arterial of oxygen,Pa O2)高于对照组(P<0.05),动脉血二氧化碳分压(arterial partial pressure of carbon dioxide,Pa CO2)低于对照组(P<0.05),但两组p H比较差异无统计学意义(P>0.05);治疗前两组氧合指数(oxygenation index,OI)比较差异无统计学意义(P>0.05);治疗2 h、6 h、12 h、24 h、48 h后,观察组OI均低于对照组(P<0.05);观察组并发症发生率低于对照组(P<0.05)。结论:HFOV联合PS治疗NRDS效果明显,可快速改善患儿的氧合功能,降低并发症的发生,值得临床推广。 Objective:To observe the effects of high frequency oscillatory ventilation( HFOV) combined with pulmonary surfactant( PS) on the oxygenation function of neonate respiratory distress syndrome( NRDS). Methods: A total of 164 cases of patients with NRDS were randomly divided into observation group and control group,82 cases in each group. The patients in observation group received HFOV combined with PS and those in control group received conventional mechanical ventilation( CMV) with PS. The clinical efficacy of the two groups was compared. Results: After treatment,Pa O2 in observation group was significantly higher than that in control group( P〈0. 05),and Pa CO2 was significantly lower than that in control group( P〈0. 05),but there was no significant difference in p H between the two groups( P〈0. 05). Before treatment,there was no significant difference in oxygenation index( OI) between the two groups( P〈0. 05). After 2 h,6 h,12 h,24 h and 48 h treatment,OI in observation group was significantly lower than that in control group( P〈0. 05). The incidence of complications in observation group was significantly lower than that in control group( P〈0. 05). Conclusion: HFOV combined with PS in the treatment of NRDS has significant effects,and can quickly improve oxygenation function and reduce the incidence of complications. It is worthy of clinical promotion.
