摘要
目的:研究中药熏洗方联合通理汤治疗痔疮PPH术后并发症的临床疗效。方法:本次观察病例我院肛肠科混合痔PPH术后80患者,按入院顺序随机分为对照组与观察组。两组均行PPH手术,术后对照组予以常规西药灌肠以及高锰酸钾熏洗,观察组服用自拟方通理汤煎剂配合中药熏洗。对比两组术后疼痛程度、治疗疗效、术后炎症消退天数和愈合天数,并进行肛周直肠超声检查。结果:观察组总有效率95%,显著高于对照组(85%),复发率和不良反应显著低于对照组(P<0.05)。治疗后观察组舒张半径与静息半径差值(R)显著高于对照组(P<0.05)。观察组术后8小时、48小时以上疼痛程度与对照组差异不显著,在24小时后疼痛指数明显低于对照组(P<0.05)。观察组伤口愈合天数明显低于对照组(P<0.01)。结论:中药熏洗方联合通理汤治疗痔疮PPH术后并发症如疼痛、炎症等有较好防治功能,且能改善肛门功能,值得临床推广。
Objective ： Studying the clinical effect of combined treatment of traditional Chinese medicine fumigation decoction combined with Tongli Decoction in the treatment of postoperative complications of hemorrhoids PPH. Methods ： Selecting 80 cases of patients with anorectal postoperative mixed hemorrhoids after PPH in our hospital. They were randomly divided into two groups according to admission order, which were controlled group and observation group respectively. PPH operation was performed in the two groups. After operation, the controlled group was given routine western medicine enema and Potassium Permanganate fumiga- tion, the observation group taking Tongli Decoction combined with herbal fumigation. Compared with the two groups of postoperative pain degree, the treatment effect, the number of postoperative inflammation subsided and the healing days, and the rectal ultrasound examination. Results： The total effective rate of the observation group was 85%, which was significantly higher than that of the con- trolled group, which was 95%, and the recurrence rate and adverse reaction were significantly lower than that of the controlled group （P 〈 0. 05 ）. In the rectal examination, the difference between the diastolic radius and the resting radius （R） of the obser- vation group was significantly higher than that of the controlled group （ P 〈 0. 05 ）. The degree of drug pain of the observation group was not significantly different from the controlled group at 8 hours and 48 hours after operation, and the pain index was significantly lower than that of the controlled group （ P 〈 0. 05 ） after 24 hours. The observation group was significantly lower than that of the controlled group （P 〈 0. 01 ） in the number of days of diminishing inflammation and healing days. Conclusion： Traditional Chinese medicine fumigation decoction combined with the treatment of hemorrhoids PPH postoperative complications such as pain, inflamma- tion and so on have better control function, and can improve the anal function, it is worthy of clinical promotion.
