瑞格列奈对初诊2型糖尿病患者糖脂代谢的影响 被引量:19

Effect of repaglinide on glucose and lipid metabolism in patients newly-diagnosed as type 2 diabetes mellitus

摘要 目的探讨瑞格列奈对初诊2型糖尿病患者血糖、血清真胰岛素、TG和TC的影响。方法 50例初诊2型糖尿病患者随机分为瑞格列奈组27例和格列齐特组23例,口服相应降糖药物4周。治疗前后测定两组患者空腹、餐后30 min及餐后2 h的血糖和血清真胰岛素、空腹TG和TC水平,计算早期胰岛素分泌指数;对瑞格列奈组治疗后空腹血糖(FBG)、空腹血清真胰岛素、TC、TG改变值间的相关性进行分析。结果治疗后两组的FBG、餐后30 min血糖及餐后2 h血糖均较前下降(P<0.05),但两组间比较,差异无统计学意义(P>0.05)。治疗后,除外格列齐特组空腹时,两组空腹、餐后30 min及2 h的血清真胰岛素水平均较前升高(P<0.05),但两组间比较,差异无统计学意义(P>0.05);治疗后两组胰岛素分泌指数均较前升高(P<0.05),且瑞格列奈组高于格列齐特组(P<0.05)。治疗后,瑞格列奈组的空腹TG和TC均较治疗前下降(P<0.05),而格列齐特组与治疗前比较,差异均无统计学意义(P>0.05)。瑞格列奈组患者空腹TC的降低与空腹真胰岛素分泌的增加呈正相关(P<0.05)。结论瑞格列奈对初诊2型糖尿病患者的血糖控制疗效较好,并有效改善早相胰岛素分泌及血脂水平,对延缓患者心血管动脉粥样硬化有益。 Objective To investigate the effect of repaglinide on blood glucose,serum insulin,triglyceride,and total cholesterol in patients newly diagnosed as type 2 diabetes mellitus. Methods Fifty patients newly diagnosed as type 2 diabetes mellitus were divided into repaglinide group( n = 23) or gliclazide group( n = 27),and were administered with corresponding oral hypoglyceimic agents for 4weeks. The fasting,30 min postprandial and 2h postprandial blood glucose,true insulin,and fasting triglyceride( TG) and total cholesterol( TC) were measured in both groups before and after treatment,and early insulin secretion index was calculated. The correlation analysis was conducted among change values of fasting blood glucose( FBG),fasting true insulin,TC and TG. Results After treatment,the levels of FBG,30 min postprandial and 2h postprandial blood glucose in both groups decreased compared to those before treatment( P〈0. 05),but no statistical difference was found between the two groups( P〈0. 05). After treatment,the levels of fasting( except for gliclazide group),30 min postprandial and 2h postprandial true insulin in both groups increased compared to those before treatment( P〈0. 05),but no statistical difference was found between the two groups( P〉0. 05). After treatment,the early insulin secretion indices in both groups decreased compared to those before treatment( P〈0. 05),with the indices higher in repaglinide group( P〈0. 05). After treatment,the levels of fasting TG and TC in the repaglinide group decreased( P〈0. 05),but no significant change in the levels of fasting TG and TC was observed in the gliclazide group( P〉0. 05). The decrease of fasting TG positively correlated to the increase of fasting true insulin in the repaglinide group( P〈0. 05). Conclusion For the patients newly diagnosed as type 2 diabetes mellitus,repaglinide can not only obtain a good efficacy of blood control,but also improve effectively the early-phase insulin secretion and blood lipid level,and is helpful for slowing cardiovascular atherosclerosis.
