摘要
目的观察早期肠内营养对减轻重型颅脑损伤患者应激及改善预后的效果。方法选取我院急诊科2015年7月至2016年7月期间收治的重型颅脑损伤患者90例作为研究对象,根据予以患者肠内营养时间的不同分为两组各45例,其中观察组患者在入院后12 h内进行肠内营养支持,对照组患者在入院后48 h后予以肠内营养支持,观察两组患者入院时和治疗后72 h的AGML发生率,SBP、NEU、WBC、Glu水平,以及GCS评分;观察两组患者的恢复清醒时间,以及治疗后1个月的NIHSS评分和死亡率。结果治疗72 h后,两组患者的SBP、NEU、WBC、Glu水平和GCS评分均较入院时有显著改善,且观察组的各项指标均显著优于对照组(P<0.05)。观察组患者的恢复清醒时间显著少于对照组,治疗后1个月的NIHSS评分和死亡率均显著低于对照组,差异均有统计学意义(P<0.05)。结论早期肠内营养支持可充分满足重型颅脑损伤患者的营养和机体代谢的需求量,可有效减轻患者的应激状态,改善患者的预后情况。
Objective To observe the effect of early enteral nutrition to ease the stress and improve the prognosis of patients with severe craniocerebral injury. Methods 90 cases of patients with severe craniocerebral injury admitted to our department from July 2015 to July 2016 were selected. According to the start time of enteral nutrition, all cases were divided into two groups. The observation group received enteral nutrition within 12 h after admission, while the control group received enteral nutrition over 48 hours after admission. At admission and 72 hours after admission, the incidence of AGML, the levels of SBP, NEU, WBC and Glu, and GCS score were recorded. The time of consciousness recovery, and NIHSS score and mortality at 1 month after treatment were observed. Results 72 hours after treatment, both groups had significant improvement of levels of SBP, NEU, WBC and Glu, and GCS score compared with those at admission, and the indicators of observation group were significantly better than those of control group, with statistical differences (P 〈0.05). The time of consciousness recovery of observation group was significantly shorter than that of control group, and the NIHSS score and mortality at 1 month after treatment were significantly lower than those of control group, with statistical differences (P 〈0.05). Conclusions Early enteral nutrition can fully meet the demand of nutrition and metabolism of patients with severe craniocerebral injury, which can effectively ease the stress and improve the prognosis of patients.
