摘要
目的 探讨前瞻性目视管理法在重症脑卒中患者早期肠内营养并发症防护中的应用效果.方法 将前瞻性目视管理法实施前(2015年1~6月)和实施后(2015年7~12月)收治于我院的重症脑卒中行早期肠内营养治疗的患者各54例作为研究对象,实施前后分别设为对照组和试验组.对照组接受常规早期肠内营养护理,试验组在对照组基础上接受针对早期肠内营养并发症的前瞻性目视管理法干预,对两组重症脑卒中患者干预后的相关指标进行比较.结果 试验组重症脑卒中患者干预后的早期肠内营养相关性并发症发生率显著低于对照组,干预后的营养状况指标(血清总蛋白、白蛋白、血红蛋白、血糖)均显著优于对照组,目标喂养量所需时间、需接受重症监护的时间均显著短于对照组,差异有统计学意义(P<0.05).结论 将前瞻性目视管理法应用于重症脑卒中患者早期肠内营养并发症防护护理实践之中,可显著降低其肠内营养相关性并发症发生概率,实现高效营养支持目标,缩短接受重症监护的时间.
Objective To explore the application effect of prospective visual management on the early enteral nutrition complications of patients with severe stroke. Methods A total of 54 cases of severe stroke patients who re-ceived the early enteral nutrition therapy before the implementation of prospective visual management ( from January 2015 to June 2015 ) and a total of 54 cases of severe stroke patients who received the early enteral nutrition therapy after the implementation (from July 2015 to December 2015) in our hospital were selected as the research objects. The cases before and after the implementation were taken as the control group and experimental group respectively. The control group received conventional enteral nutritional care, while the experimental group received the visual management meth-od targeted for early enteral nutrition complications on the basis of the control group. The relevant indicators of severe stroke patients after the intervention of the two groups were compared. Results The incidence of the early enteral nutri-tion complications of patients with severe stroke after intervention of the experimental group was significantly lower than that of the control group. The nutritional status indicators ( serum total protein, albumin, hemoglobin, blood sugar) af-ter intervention of the experimental group were significantly better than that of the control group. The amount of time re-quired to achieve the goal of feeding, the time required for intensive care were significantly shorter than that of the con-trol group ( P<0. 05 ) . Conclusions To implement prospective visual management on the prevention of early enteral nutrition complications of patients with severe stroke protection can significantly reduce the incidence of enteral nutrition-related complications, realize efficient nutrition goals, and shorten the intensive care time.
