摘要
目的比较Herbert中空螺钉与锁骨重建钢板治疗成人锁骨中段移位骨折的临床效果。方法选取苏州大学附属常州肿瘤医院骨科2015年6月~2016年2月锁骨中段骨折患者30例,其中15例行Herbert中空螺钉治疗(HS组),15例行锁骨重建钢板治疗(RP组)。比较两种治疗方式的手术时间、出血量、皮肤切口长度、并发症发生情况,术后满意度,按照视觉模拟评分(VAS)、肩关节的Constant评分评估临床疗效,同时行影像学观察。结果两组的手术切口长度、手术时间、术中平均失血量结果比较,差异有统计学意义(P<0.05)。随访时术后1周两组VAS比较,差异无统计学意义(P>0.05)。末次随访时,两组肩关节的Constant评分结果比较,差异无统计学意义(P>0.05)。HS组患者手术满意率为93.3%,RP组患者手术满意率为73.3%,差异有统计学意义(P<0.05)。结论 Herbert中空钉治疗锁骨中段骨折的总体疗效与锁骨重建钢板相当,但前者创伤小、术中失血少、手术时间短,满足患者对美观的要求,操作简便安全,是治疗锁骨中段移位骨折的有效方法。
Objective To compare the clinical effects of internal fixation for mid-shaft clavicle fracture using Herbert cannulated screw or reconstruction plate. Methods From June 2015 to February 2016, in Department of Orthopaedics, Changzhou Cancer Hospital of Soochow University, 30 patients with clavicular fractures were selected, there were 15 cases with Herbert cannulated screw fixation (HS group), 15 cases with reconstruction plate fixation (RP group). Operative time, blood loss and length of incision were observed, and the posoperative complications, the posoperative satisfaction, the visual analogue scale (VAS), the Constant score and imaging outcomes were compared between two groups. Results The blood loss, length of incision and operative time in two groups were compared, the differences were statistically significant (P 〈 0.05). There were no significant difference in the VAS scores at 1 weeks after surgery (P 〉 0.05). at the last follow-up, in the Constant score between tow groups were compared, the difference was not statistically significant (P 〉 0.05). The satisfaction of HS group was 93.3%, which of the RP group was 73.3%, those of tow groups were compared, the difference was statistically significant (P 〈 0.05). Conclusion Herbert cannulated bone screw fixation has the same effectiveness as reconstruction plate fixation in treatment mid-shaft clavicle fracture, But the former has less invasive, shoter time and little hemorrhage. Therefore, the system appears to be a useful and effective manner to treatment of mid-shaft clavicle fractures.
