摘要
目的:探讨腰椎后路经多裂肌间隙入路治疗腰椎间盘突出症(LDH)对腰腿痛和功能障碍的影响。方法:选取2014年1月至2015年6月四川省广元市中心医院收治的87例LDH患者随机分为治疗组(45例)和对照组(42例),对照组给予传统腰椎后路正中入路手术治疗,治疗组给予腰椎后路经多裂肌间隙入路手术治疗,对比两组临床效果。结果:与对照组比较,治疗组患者手术时间和住院时间较短,术中出血量较少,术后引流量较多,差异均具有统计学意义(均P<0.05);两组术前腰腿痛视觉模拟评分(VAS)及Oswestry功能障碍指数评分(ODI)比较,差异均无统计学意义(均P>0.05),术后6个月,两组VAS、ODI评分均较术前明显降低(均P<0.05),且治疗组VAS、ODI评分均明显低于对照组(均P<0.05);治疗组术后并发症总发生率为4.44%,明显低于对照组的21.43%(P<0.05)。结论:腰椎后路经多裂肌间隙入路治疗LDH可有效缓解患者腰腿痛症状,改善功能障碍,具有手术创伤小、术后恢复快、并发症少的优势。
Objective： To investigate the clinical effect of posterior lumbar multifidus muscle approach on lumbocrural pain and dysfunction in lumbar disc herniation （LDH） patients. Methods： 87 patients with LDH treated in our hospital from January 2014 to June 2015 were selected and randomly divided into a treatment group （ n=45） and a control group （ n =42）. The patients in the control group received tradi- tional posterior approach surgery, while those in the treatment group received posterior lumbar multifidus muscle approach. The clinical effect was compared between the two groups. Results： The operation time, intraoperative blood-loss volume, postoperative drainage volume and hospital days were obviously lower in the treatment group than those in the control group （ P〈0.05）. There were no significant differences in the scores of visual analogue scale （VAS） and Oswestry functional disability index （ODI） before operation between the two groups （ P〉0.05）. At 6-month after operation, the VAS scores and ODI were decreased in both groups, and they were more significant decreased in the treatment group （ P〈0. 05）. The inci- dence of complications in the treatment group was 4.44%, which was lower than that in the control group （21.43%） （P〈0.05）. Conclusion： Application of posterior lumbar multifidus muscle approach in the treatment of LDH could effectively relief lumbocrural pain, with advantages of small surgical trauma, speedy recovery and less complications.
