摘要
目的研究窒息新生儿的血糖及电解质的变化情况及其临床意义。方法 289例窒息新生儿根据Apgar评分分为重度窒息组124例,轻度窒息组165例,并以同期健康新生儿165例为对照组,比较3组研究对象血糖及电解质的检测情况。结果轻度窒息组低血糖的发生率为22.4%,重度窒息组高血糖的发生率为39.5%,低血糖的发生率为12.9%。轻度窒息组、对照组、重度窒息组血糖依次升高,差异有统计学意义(P<0.01);对照组、轻度窒息组、重度窒息组血Na^+、Cl^-、Ca^(2+)浓度依次降低(P<0.01);窒息患儿血Na^+、Cl^-、Ca^(2+)与Apgar评分均呈正相关(r=0.657、0.698、0.752,P<0.05)。结论临床工作者需密切关注窒息新生儿的血糖及电解质紊乱情况,积极治疗,以降低新生儿窒息的致死、致残率。
Objective To research the changes of blood glucose and electrolytes in neonates with asphyxia and the clinical significance. Methods A total of 289 neonates with asphyxia were divided into severe asphyxia group ( 124 neonates) and mild asphyxia group (165 neonates) according to Apgar score; 165 healthy neonates during the same period were selected as control group. Blood glucose levels and electrolytes were detected and compared in the three groups. Results The incidence rate of hypoglycemia in mild asphyxia group was 22.4% , the incidence rate of hyperglycemia in severe asphyxia group was 39.5% , the incidence rate of hypoglycemia in severe asphyxia group was 12. 9%. Blood glucose levels in mild asphyxia group, control group and severe asphyxia group increased gradually, there was statistically significant difference (P〈0. 01 ) . The concentrations of Na+, Cl-, and Ca2+ in mild asphyxia group, control group and severe asphyxia group decreased gradually, there was statistically significant difference (P〈0.01) . The concentrations of Na+, Cl-, and Ca2+ in neonates with asphyxia were positively correlated with Apgar score ( r = 0. 657, 0. 698, 0. 752, P〈0. 05) . Conclusion Clinical workers should pay close attention to blood glucose and electrolyte disturbance in neonates with asphyxia, active treatment shouht be taken to reduce the mortality rate and disability rate of neonates with asphyxia.
