摘要
目的观察比较开环式微创痔吻合术(TST术)与外剥内扎术(Milligan-Morgan术)治疗脱垂性痔病的临床效果。方法选取本院自2013年2月至2015年12月间收治的脱垂性痔病患者150例,随机分为TST组和M-M组(每组各75例),分别采用TST术和M-M术进行治疗。对两组患者术中出血量、手术时间和术后并发症发生率以及治疗有效率等指标进行观察比较。结果TST组手术时间(32.11±1.52)min和术中出血量(11.44±3.36)m L明显少于M-M组(41.00±3.61 min,55.64±5.38 m L)(P﹤0.05);TST组术后水肿、出血、尿潴留等并发症发生率(8.00%)明显低于M-M组(24.00%)(P﹤0.05);TST组的治疗总有效率(97.33%)显著高于M-M组(84.00%)(P﹤0.05)。结论相比于传统的M-M术,TST治疗脱垂性痔病可有效减少患者术中出血和手术时间,减少并发症发生,治疗效果良好,具有一定的临床应用价值。
Objective To compare the effect of TST and Milligan-Morgan surgery in patients with prolapsed hemorrhoids.Methods150 patients with prolapsed hemorrhoids treated in our hospital between February 2015 and September 2015 and selected and randomized to receive TST or Milligan Morgan Surgery group using random number table.TST group received open-loop minimally invasive hemorrhoid anastomosis.Milligan Morgan Surgery group received Milligan Morgan Surgery.We compared between the two groups in terms of amount of bleeding during operation,operation duration,postoperative complication rate and mortality.Results The operationduration of TST group（32.1±1.5） min and amount of bleeding（11.4±3.3） mL were significantly lower than Milligan-Morgan Surgery group（41.0±3.6 min,55.6±5.3 mL,respectively,P﹤0.05）.The rate of Postoperative complications,such as edema,hemorrhage,urinary retention,in TST group（8%） was significantly lower than the Milligan-Morgan Surgery group（24%,P〈0.05）.Total effectiveness rate in TST group（97.33%） was significantly higher than Milligan-Morgan Surgery group（84%,P〈0.05）.Conclusions Compared with traditional Milligan-Morgan Surgery,TST could reduce amount of bleeding and operation duration,reduce the incidence of postoperative complications and improve the treatment efficiency in patients with prolapsed hemorrhoids.
