摘要
目的探析小切口切扩引流术与A型肉毒素注射对肛裂患者肛门外形及功能的保护作用。方法选取2012年5月至2016年5月我院150例肛裂患者作为研究对象,随机分为两组,每组各75例。观察组采用小切口切扩引流术与A型肉毒素注射,传统组采用肛管内括约肌切断+肛裂切除术。比较两组术口相关情况、治疗效果及术后并发症发生情况。结果观察组切口愈合时间为(5.76±1.04)d,切口愈合面积为(2.08±0.19)cm^2,疼痛评分为(1.98±0.64)分,均分别显著低于传统组,差异有统计学意义(P<0.05)。观察组痊愈率为84.00%、总体有效率为98.67%,传统组的痊愈率占68.00%、总体有效率为90.67%,两组痊愈率及总体有效率差异均有统计学意义(P<0.05)。传统组并发症总体发生率为12.00%,显著高于观察组,差异有统计学意义(P<0.05)。结论与肛管内括约肌切断+肛裂切除术相比,小切口切扩引流术与A型肉毒素注射治疗肛裂的疗效更优,利于保护肛门功能,安全性良好。
Objective To investigate the effect of small incision drainage with botulinum toxin type A injections for patients with anal fissure.Methods 150 patients who were treated in our hospital from May 2012 to May 2016 were recruited as study subjects.They were randomly assigned to two groups,with 75 cases in each group.Patients in treatment group received small incision drainage and injection of botulinum toxin type A,and the control group received anal sphincter and anal fissure resection.The therapeutic effectiveness and postoperative complications were compared between the two groups.Results Time to wound healing,area of wound healing and the pain score in the treatment group after operation were(5.76±1.04) day,(2.08±0.19) cm^2,and(1.98±0.64) points,respectively,which were significantly lower than those in the control group(P〈0.05).The curative rate and total effective rate of the treatment group were 84% and 98.67%,respectively,which were significantly higher than those in the control group(68% and90.67%,respectively,P〈0.05).The incidence of complications in the control group was 12%,which was significantly higher than thatin the treatment group(P〈0.05).Conclusion Small incision drainage with botulinum toxin type A injections is effective for the treatment of anal fissure.It could reduce pain,protect anal function and is a safe procedure.
