FeNO和血清总IgE在ACOS和COPD患者中的表达及临床意义 被引量:11

摘要 目的探讨呼出气一氧化氮(FeNO)、血清总IgE在哮喘-慢性阻塞性肺疾病重叠综合征(ACOS)及COPD患者中的表达及临床意义。方法收集诊治的ACOS患者42例、COPD患者178例(根据舒张试验分为阳性组50例,阴性组128例)、慢性咳嗽患者52例为研究对象,比较4组患者肺通气功能、FeNO、血清总IgE的差异。结果肺通气功能指标(FEV1/预计值、FVC/预计值、FEV1/FVC)在ACOS组、COPD舒张试验阳性组、COPD舒张试验阴性组中均低于慢性咳嗽组(P=0.000),3组间比较差异无统计学意义(均P>0.05)。Fe NO、血清总Ig E表达水平为ACOS组>COPD舒张试验阳性组>COPD舒张试验阴性组>慢性咳嗽组(均P=0.000)。相关分析显示4组患者的FeNO、血清总Ig E与肺通气功能指标(FEV1/预计值、FVC/预计值、FEV1/FVC)均无显著相关性(P>0.05)。结论FeNO、血清总IgE在ACOS患者中表达增高,有望成为诊断ACOS的客观指标;FeNO、血清总IgE升高及舒张试验阳性的COPD患者需注意ACOS的存在。 Objective To investigate the expression and clinical significance of exhaled nitric oxide (FeNO) and serum total IgE in patients with asthma-COPD overlap syndrome (ACOS) or chronic obstructive pulmonary disease (COPD). Methods Forty -two patients with ACOS (ACOS group) and 178 patients with COPD including 50 patients in diastolation positive group and 128 patients in diastolation negative group according to bronchial diastolation test as well as 52 patients with chronic cough (chronic cough group) who were treated in our hospital from September 2014 to September 2015 were enrolled in the study.The pulmonary ventilation function of patients , FeNO and serum total IgE levels were observed and compared among the four groups.Results The pulmonary ventilation function parameters including FEV 1 /prediction value %,FVC/prediction value %,FEV1/FVC in ACOS group, diastolation positive group and diastolation negative group were significantly lower than those in chronic cough group ( P 〈0.01),however,there were no significant differences among the former three groups ( P 〉0.05).The expression levels of FeNO and serum total IgE were ACOS group 〉diastolation negative group 〉diastolation positive group 〉chronic cough group ( P 〈0.01).The correlation analysis showed that there no obvious correlation between FeNO,serum total IgE levels and pulmonary ventilation function parameters including FEV 1/prediction value %,FVC/prediction value %,FEV1 /FVC ( P 〉0.05).Conclusion The expressin levels of FeNO and serum total IgE are increased in patients with ACOS,which may become objective indexes to diagnose ACOS .The patients with the increase of FeNO and /or serum total IgE,or COPD patients with diastolation test being positive should pay much attention to occurrence of ACDS.
