摘要 目的探索肱骨骨折术后骨不连的危险因素。方法回顾性分析2012年1月-2016年10月在我院创伤骨科住院的肱骨骨折术后患者病例资料。纳入20例肱骨骨折术后骨不连患者,根据骨不连组患者的年龄和性别,按1∶2配比选择同等条件的骨折愈合的患者(对照组,40例),分析肱骨骨折术后骨不连的危险因素。结果单因素分析显示,吸烟、饮酒史和骨折位置可能影响肱骨骨折骨不连(P<0.05)。多因素回归分析显示,骨折位置(OR=4.58,P=0.03)是肱骨骨折术后骨不连的独立危险因素。结论左侧肱骨骨折是肱骨骨折术后骨不连的独立危险因素,有吸烟和饮酒史的左侧肱骨骨折患者应注意手术后发生骨不连的可能性。 Objective To analyze and identify the risk factors for nonunion among patients with humeral fractures undergoing surgical fixation.Methods We retrospectively collected all patients with humeral fractures undergoing surgical fixation between January 2012 and October 2016 in department of orthopaedic trauma.Twenty humeral nonunion were identified.Each nonunion case was matched by age and sex with two controls of healed fractures(40 control patients).Results In univariate analysis, left humeral fractures, history of smoking and history of drinking, were associated with a higher risk of nonunion(P〈0.05).In multivariate analysis, side of fracture(left)(OR=4.58, P=0.03) was the independent risk factor for nonunion of humeral fracture after surgical fixation.Conclusion Left side of fracture was independent risk factor for nonunion of humeral fracture after surgical fixation.Patients who have history of smoking or drinking, and left-side fracture should be counseled about the possibility of nonunion after surgical fixation.
