多模态影像检查及组合脑电监测在难治性颞叶癫痫外科治疗中的应用 被引量:4

The application of multi-modality imaging combined with nerve electrophysiological examination in the surgical treatment of intractable temporal lobe epilepsy

摘要 目的 探讨多模态影像学检查结合多种脑电监测技术在难治性颞叶癫痫(temporal lobe epilepsy,TLE)外科治疗中的应用价值。方法 收集2009年1月至2015年9月在我院神经外科手术治疗的TLE患者59例,采用多模态影像学检查并根据需要组合多种神经电生理技术辅助定位致癫灶,给予个体化手术,统计患者临床资料及术后疗效情况。结果 59例患者均手术顺利,无死亡病例,1例患者术后出现短暂性言语不利,1例患者术后发生脑血管痉挛导致偏瘫,1例患者出现薄层硬膜下血肿,予对症治疗后均恢复;术后随访13个月-3年,Engel分级Ⅰ级48例,Ⅱ级8例,Ⅲ级3例。结论 多模态影像学结合多种神经电生理技术更有助于精确定位癫痫灶,针对不同病情指导实施个体化手术,能够提高难治性TLE外科手术成功率和临床疗效。 Objeetive To explore the application value of multi-modality imaging corn bined with nerve electrophysiological technique in the surgical treatment of intractable temporal lobe epilepsy(TLE). Methods Admitted into 59 TLE patients from Jan 2009 to Sep 2015,all of them took multi--modality imaging and nerve eleetrophysiologieal examination to locate the epi- leptogenic focus,and then had an operation individually. Results In this study ,all of 59 cases were operated suceessfully with no death ease, while 3 eases had postoperative complication who recovered after treatment;According to Engel's class, there were 48 eases of grade Ⅰ 78 eases of grade Ⅱ ,3 eases of grade Ⅲ ,and no case of grade Ⅳ in follow-up with 13 months to 3 years. Conclusion The muhi--modality imaging combined with neural eleetrophysiologieal examination can improve the success rate and clinical efficacy of refractory TLE surgery by helping to accu rately locate the epileptic focus and design individual operation plan.
