摘要
目的探讨体重指数、腰臀比及皮下脂肪厚度与糖尿病肾病发病率的相关性。方法选取河北省石家庄市第二医院2013年8月~2015年8月就诊的糖尿病患者842例分为三组,即肥胖组(BMI>28 kg/m2)、超重组(BMI为24~28 kg/m2)及正常组(BMI<24 kg/m2)。空腹12 h后取血,对空腹血糖(FBG)、三酰甘油(TG)、糖化血红蛋白(Hb A1c)、总胆固醇(TC)、低密度脂蛋白(LDL-C)、肌酐(Cr)、高密度脂蛋白(HDL-C)、尿素氮(BUN)等指标进行检查,测定微量白蛋白(UMA),并行眼底检查筛查糖尿病视网膜病变,并检测体重指数(BMI)、腰臀比(WHR)及皮下脂肪厚度(SFT)水平。结果三组FBG、Hb A1c、TG、TC、HDL-C、LDL-C、BUN、Cr、UMA比较,差异无统计学意义(P>0.05)。肥胖组糖尿病肾病患病率最高,超重组次之,正常组最低,差异有高度统计学意义(P<0.01)。肥胖组BMI、WHR及SFT值最高,超重组次之,正常组最低,差异有高度统计学意义(P<0.01)。BMI、WHR及SFT与糖尿病肾病患病率呈正相关(r=0.081、0.138、0.087,P<0.05)。Logistic回归分析结果发现,SFT是糖尿病肾病的显著性影响因素(P<0.05)。结论 BMI、WHR及SFT与糖尿病肾病患病率呈正相关。
Objective To investigate correlation between BMI, WHR, SFT and incidence rate of diabetic nephropathy. Methods 842 patients with diabetes mellitus in the Second Hospital of Shijiazhuang City, Hebei Province from August 2013 to August 12015 were selected and divided into three groups: obesity group (BMI 〉 28 kg/m2), overweigh group (BMI was from 24 to 28 kg/m2) and normal group (BMI 〈 24 kg/m2). Blood was detected after fasting 12 h, FBG, TG, HbAlc, TC, LDL-C, Cr, HDL-C, BUN and other indicators were checked. Microalbumin (UMA) was detected. Diabetic retinopathy was screened by using fundus examination. BMI, WHR, SFT levels were detected. Results FBG, HhA1c, TG, TC, HDL-C, LDL-C, BUN, Cr, UMA among three groups were compared, with no statistical difference (P 〉 0.05). The incidence rate of diabetic nephropathy was highest in obesity group, overweigh group was the second, and normal group was the lowest, the difference was statistically significant (P 〈 0.01). BMI, WHR, SFT were highest in obesity group, overweigh group was the second, and normal group was the lowest, the difference Was statistically significant (/9 〈 0.01). BMI, WHR and SFT were positively correlated with incidence rate of diabetic nephropathy (r = 0.081, 0.138, 0.087, P 〈 0.05). Logistic regression analysis showed that SFT had significant influence on the pathogenesis of diabetic nephropathy (P 〈 0.05). Conclusion BMI, WHR, SFF are positively correlated with incidence rate of diabetic nephropathy.
