摘要
目的:探讨惠州市第二妇幼保健院(以下简称"我院")新生儿重症监护病房(neonatal intensive care unit,UICU)患儿治疗用药情况,为新生儿科临床合理用药提供依据。方法:统计分析2014—2016年我院NICU收治的新生儿患者的用药情况,计算各种治疗药物的使用频率,分析药物使用方法是否与药品说明书一致、特殊使用类抗菌药物的应用是否合理及不良反应发生情况。结果:2014—2016年,我院NICU治疗新生儿疾病的总用药量有所增加,用量排序居前3位的分别为营养药、消化系统用药和血液系统用药;有38种药品的说明书中未提及可供新生儿使用;特殊使用级抗菌药物存在使用时间偏长、用药起点偏高等情况;发生药品不良反应病例数排序居前3位的分别为抗菌药物、心血管系统用药、中枢神经系统用药。结论:我院NICU使用的药物品种及用药特点比较明确;存在超说明书用药现象;抗菌药物尤其是特殊使用级抗菌药物的使用存在不合理情况,应加强干预;应加强药品不良反应监测,保证NICU合理用药。
OBJECTIVE:To probe into the drug use in children of neonatal intensive care unit(UICU) in Huizhou the Second Maternal and Child Health Hospital( hereinafter referred to as "our hospital"), so as to provide reference for the rational drug use for neonatology department in clinic. METHODS: Statistical analysis was conducted on the drug use in children of UICU during 2014-2016, the frequency of use was calculated ; in addition, whether the drug use method was consistent with the description, whether the use of special antibiotics was rational and the incidence of adverse drug reactions were analyzed. RESULTS: During 2014-2016, the total amount of drugs used for neonatal diseases increased significantly in our hospital, the top three drugs were respectively nutriceutical, digestive system agents and blood system drugs. The specification of 38 kinds of drugs did not mention for use in the newborn. Special antibiotics had problems of rather long time in drug use and high starting treatment. The top three drugs ranked by number of adverse drug reactions were respectively antibiotics, cardiovascular drugs and central nervous system drugs. CONCLUSIONS: The drug use categories and characteristics in NICU of our hospital is relatively clear. Yet there are still some off-label drug use phenomenon. The use of antibiotics especially special antibiotics is basically irrational, the intervention should be strengthened; and the adverse drug reactions monitoring are supposed to be strengthened so as to ensure medication safety in NICU.
