摘要
目的检测血清总胆汁酸(TBA)水平,探讨血清总胆汁酸测定在足月新生儿黄疸诊治中的临床意义。方法选取82例临床资料完整,诊断为新生儿黄疸的患儿为研究对象,分为重度黄疸组(A组)、轻度黄疸组(B组)、生理黄疸组(C组),检测各组患儿血清中总胆汁酸(TBA)的水平及其他肝功能相关指标并进行比较。结果 1A组及B组TBA、TBTL(总胆红素)、DBTL(直接胆红素)、ALT(丙氨酸转氨酶)水平显著高于C组,差异有统计学意义(P<0.05);B组与C组GGT(γ-谷氨酰转肽酶)水平比较差异无统计学意义(P>0.05)。2A,B两组TBA水平与TBTL,DBTL,GGT,ALT各项肝功能指标有直线相关关系。3A,B组治疗前TBA,TBTL水平较治疗后明显下降,差异有统计学意义(P<0.05),但A组TBA及TBIL消退时间较B组长,差异有统计学意义(P<0.05)。结论血清总胆汁酸测定在新生儿黄疸的诊断中具有重要临床意义,有利于新生儿黄疸的早期诊治及预后。
Objective To explore the clinical significance of serum total bile acid (TBA) determination in the diagnosis and treatment of full-term neonatal jaundice. Methods Eighty-two cases of neonatal jaundice with full clinical data were divided into severe jaun- dice group ( A group ), mild jaundice group { B group ), and physiological jaundice group ( C group ). The levels of serum total bile acid ( TBA ) and other liver function indexes in each group were detected and compared. Results (1)The levels of TBA, TBTL ( total bilirubin ), DBTL( di- rect bilirubin ) and ALT{ alanine aminotransferase) in group A and group B were higher than those of group C,the difference was statistically significant ( P 〈 0. 05) ;There was no significant difference (P 〉 0. 05) between the B group and the C group of GGT( gamma gamma Valley ammonia acyl transfer peptide ). (2)A, B two groups of TBA levels and TBTL, DBTL, GGT, ALT of the liver function indicators had linear cor- relation. (3)The levels of TBTL and TBA in A and B group were significantly decreased compared with before treatment, the difference was sta- tistically significant ( P 〈 0.05 ) , but the regression time of TBA and TBIL in A group were longer than the B group, the difference was statisti- cally significant ( P 〈 0.05 ). Conclusion The determination of serum total bile acid in the diagnosis of neonatal jaundice has important clini- cal significance, it is conducive to the early diagnosis and prognosis of neonatal jaundice.
