摘要
目的探讨床旁颅脑超声检查在新生儿颅内出血诊断中的应用价值。方法选取我院出生并于出生后3 d内接受床旁颅脑超声检查的新生儿805例,回顾性分析其中经超声诊断为颅内出血新生儿的临床资料。结果 805例新生儿中经颅脑超声检查共发现颅内出血105例,其中467例早产儿中发生颅内出血76例,发病率为16.3%;338例足月儿中发生颅内出血29例,发病率为8.6%;组间比较,足月儿颅内出血的发生率显著低于早产儿,差异有统计学意义(P<0.05)。在76例发生颅内出血的早产儿中,<32周者47例(61.8%),≥32周者29例(38.2%),差异有统计学意义(P<0.05);29例发生颅内出血的足月儿中,<40周者21例(72.4%),≥40周者8例(27.6%),差异有统计学意义(P<0.05)。新生儿颅内出血最常见的发生部位为脑室,脑实质次之,硬膜下较少见。早产儿和足月儿的出血部位、脑室出血程度比较差异均无统计学意义(χ2=2.521,P=0.534;χ2=0.081,P=0.775)。结论颅脑超声在新生儿颅内出血诊断中具有重要价值,可作为筛查新生儿早期颅内病变以及随访病变转归的首选检查手段。床旁颅脑超声方便快捷,为临床早期诊断及合理治疗提供了重要依据。
Objective To analyze the application value of bedside ultrasound in diagnosis of intracranial hemorrhage (ICH) in neonates. Methods Eight hundred and five neonates who underwent bedside ultrasound examination within 3 d after delivery in our hospital were selected. The detection results were retrospectively analyzed. Results There were 105 ICH cases altogether. For 467 premature infants, there were 76 ICH cases and the attack rate was 16.3%, for 338 full-term infants, there were 29 ICH eases and the attack rate was 8.6%. The incidence rate of ICH for full-term infants was significantly lower than that of premature infants(P〈0.05). For 76 ICH premature infants,there were 47 cases(61.8 %) less than 32 weeks,29 cases(38.2%) more than 32 weeks. For 29 ICH full-term infants, there were 21 cases (72.4%) less than 40 weeks and 8 cases (27.6%) more than 40 weeks, there was significant difference(P〈0.05 ). ICH was majorly located in ventricle, then brain parenchyma and rarely in subdural part. The bleeding position for premature and full-term infants was not significantly different(x2=2.521 ,P=0.534), and the bleeding degree of ventricle was not significantly different(x2=0.081, P=0.775 ). Conclusion The craniocerebral ultrasound has an important value in the diagnosis of ICH for neonates. It shall become the primary examination choice to screen the early intracranial lesion and disease progress for neonates. Bedside craniocerebral ultrasound is quick and convenient,it provides important evidence for clinical early diagnosis and reasonable treatment.
