摘要
目的在头颈部CTA检查中利用Test-Bolus技术估算对比剂用量,以达到合理降低对比剂用量,保证检查质量的目的。方法对63例患者行64层螺旋CT头颈部CTA检查,患者被随机分为A、B两组;A组28例,根据Test-Bolus技术获取的颈总动脉时间-密度曲线(TDC)估算出颈总动脉内最大目标CT值达到500HU时所需对比剂剂量;B组35例,采用固定对比剂剂量50ml。对比分析两组资料对比剂用量及图像质量(包括动脉内强化程度、Willis环视觉评价、锁骨下和颈内静脉同侧注射残留干扰)。结果 1)A组患者使用对比剂剂量平均为(37.5±5.5)ml,比B组患者降低25%;2)两组患者动脉内CT值在主动脉弓处、颈总动脉分叉处、颈内动脉C1段、大脑中动脉起始处均无显著性差异;3)Willis环视觉评分无明显差异;4)锁骨下同侧注射残留干扰、颈内静脉污染程度评分,A组高于B组。结论在头颈部CTA检查中利用Test-Bolus技术估算对比剂用量,可合理地降低对比剂用量,保证成像质量。
Objective In order to reduce the amount of contrast agent and to improve the quality of imagings, Test-Bolus tech- nology was used to estimate the amount of contrast agent in CTA examination of the head and neck. Methods 63 patients under- went head and neck CTA examination with 64-slice spiral CT. Patients were randomly divided into group A and group B. In group A, there were 28 patients. We estimated the contrast agent dose which was required by the CT value of the carotid artery maxi- mum target increasing to 500 HU according to the time-density curve of carotid artery obtained by Test-Bolus technology. In group B, there were 35 patients. We used 50 ml contrast agent with a fixed dose and compared the contrast agent dose and quality of im- agings in two groups, including the degree of enhancement in artery, the Willis ring visual assessment, the interference caused by remained agent in subclavian artery, and internal jugular vein because of homonymous injection. Results 1 ) In group A, the av- erage contrast agent dose used by patient was (37.5 + 5.5 ) ml, which was reduced by 25% compared with patients in group B; 2) The two groups of pationts with artery within the CT value in the aortic arieh, bifurcation of commom carotid artery, the C1 segment of internal carotid artery, the start of the middle cerebral artery had no significant differences ; 3) There were no signifi- cant diffierences in the willis ring visual assessment; and 4 ) About the score of the interference caused by remained agent in sub- clavian artery and the degree of internal jugular vein pollution, group A was higher than groups B. Conclusion Using Test-Bolus technology to estimate the amount of contrast agent in the head and neck CTA examination can reduce the amount of contrast agent and improve the quality of imagings.
