摘要
目的观察利培酮、奥氮平与阿立哌唑治疗精神分裂症的疗效及对各项代谢指标所产生的影响。方法纳入我院收治141例精神分裂症患者为研究对象,按照随机抽签法将其分为三组,各47例,A组服用利培酮,B组服用奥氮平,C组服用阿立哌唑,比较三组6个月治疗结束后临床疗效、各项代谢指标及不良反应。结果三组治疗总有效率、用药前糖脂代谢指标以及血清RBP-4、PAN、不良反应比较不具显著差异(P>0.05);用药后B组LDL-C、FPG、HDL-C、TC与TG浓度明显高于A组与C组(P<0.05);且B组PAN明显低于A组与C组,RBP-4明显高于A组与C组(P<0.05)。结论对精神分裂症采用奥氮平进行治疗,能影响糖脂代谢,并且显著降低血清APN及RBP-4浓度,而服用利培酮或者阿立哌唑则不会产生明显影响,这3类药物均能获得较好疗效,除了奥氮平在代谢方面影响较大之外,其他不良反应相当。
Objective To observe the efficacy of risperidone, olanzapine and aripiprazole in the treatment of schizophrenia and the effect on the metabolic indexes. Methods A total of 141 patients with schizophrenia in our hospital were enrolled in the study, and they were randomly divided into three groups, 47 cases in each group. The lhree groups were treated with risperidone, olanzapine and aripiprazole, respectively. The clinical efficacy, metabolic indexes and adverse reactions were compared. Results There was no significant difference among three groups in the total effective rate, glucose and lipid metabolic indexes before medication, serum RBP-4, PAN and adverse reactions (P 〉 0.05). After treatment, levels of LDL-C, FPG, HDL-C, TC and TG in group B were significantly higher than those in group A and group C (P 〈 0.05). PAN in group B was significantly lower than that in group A and group C, while RBP-4 was significantly higher than that in group A and group C (P 〈 0.05). Conclusion Application of olanzapine in the treatment of schizophrenia will affect glucose lipid metabolism, and it can significantly reduce the levels of serum APN and RBP-4 while risperidone or aripiprazole does not have obvious effect. The efficacy of all the three kinds of drugs is good. In addition, the olanzapinea has great effect on metabolism, the other adverse reactions are similar to those of other drugs.
