SPECT/CT融合显像对乳癌骨单发病灶鉴别诊断价值

VALUE OF SPECT/CT FUSION IMAGING IN DIFFERENTIAL DIAGNOSIS OF BONE SINGLE LESION IN BREAST CANCER

摘要 目的探讨^(99)Tc^m-亚甲基二磷酸盐(^(99)Tc^m-MDP)SPECT/CT融合显像对乳癌骨单发病灶的鉴别诊断价值。方法我院核医学科行全身骨显像发现单发浓聚灶的乳癌病人62例,对单发浓聚部位行^(99)Tc^m-MDP SPECT/CT融合显像,由2名有经验核医学医师对显像结果进行分析并做出诊断。结果 62例乳癌病人62个单发病灶最终诊断骨转移34例,良性病变28例。全身骨显像诊断准确率为64.5%(40/62),诊断灵敏度为70.5%(24/34),诊断特异度为57.1%(16/28);^(99)Tc^m-MDP SPECT/CT融合显像诊断准确率为93.5%(58/62),诊断灵敏度为100%(34/34),诊断特异度为85.7%(24/28);^(99)Tc^m-MDP SPECT/CT融合显像诊断准确率、灵敏度和特异度均高于全身骨显像,差异有显著性(χ~2=5.600~10.641,P<0.05)。结论 ^(99)Tc^m-MDP SPECT/CT融合显像对乳癌骨单发浓聚灶有较高的鉴别诊断的价值,对病变部位的定位及定性诊断有重要价值。 Objective To assess the value of ^99Tc^m-methylene two phosphate (MDP) SPECT/CT fusion imaging in differential diagnosis of single bone lesion in breast cancer. Methods During whole body bone scanning for patients with breast cancer, a single focus in hone was found in 62 patients. ^99Tc^m-MDP SPECT/CT fusion imaging was done on the single concentration, the results of imaging were analyzed and diagnosed by two experienced nuclear physicians. Results The final diagnosis of 62 single lesions in bone of 62 breast cancer patients was bone metastasis in 34 cases and benign lesions in 28. The diagnostic accuracy of whole body bone scan was 64.5% (40/62), the sensitivity was 70.5%(24/34), the specificity was 57.10% (16/28); the diagnostic accuracy of fusion imaging of the ^99Tc^m-MDP SPECT/CT was 93.5% (58/62) ,the sensitivity was 100% (34/34) and the specificity was 85.7% (24/28), which were higher than that of the whole body bone scan, the differences were statistically significant (χ^2 =5.600-10.641 ,P〈0.05). Conclusion The ^99Tc^m-MDP SPECT/CT fusion imaging has a high value in differential diagnosis of single focus in bone of patients with breast cancer, which has an important significance in location and qualitative diagnosis of the lesion.
