摘要
甲状旁腺功能减退症患者需要长期服用钙剂和维生素D制剂。本文分析1例甲状腺全切术后甲状旁腺功能减退症患者长期治疗过程中,未规律监测血液生化指标,也未调整钙剂及维生素D制剂剂量,出现高钙危象、肾脏钙质沉着和肾功能损害等维生素D中毒表现,经过水化、利尿、降钙素治疗后,血钙降至正常。本病例报告旨在提醒临床医师重视罕见的维生素D中毒现象,并关注其治疗措施。
Patients with hypoparathyroidism need to receive long-term treatment with calcium and vitamin D supplements. We reported a case of vitamin D intoxication in a patient with hypoparathyroidism after total thyroidectomy. She presented with hypercalcemic crisis, nephrocalcinosis, and renal function impairment, because serum and urinary calcium level were not monitored during treatment and the dose of calcium and vitamin D supplements were not adjusted accordingly. Hypercalcemic crisis was alleviated with treatment of fluid replacement, furosemide, and calcitonim. Rare vitamin D intoxication should be taken seriously and treated properly.
