摘要
目的观察右美托咪定在高血压病患者术后镇痛中的疗效及安全性。方法选择2014年7月至2016年7月接受择期全麻手术后的80例高血压患者,随机分为对照组和研究组各40例。采用静脉镇痛泵自控镇痛(PCIA)给予对照组患者适当强度阿片类药物,研究组患者则在此基础上给予右美托咪定。比较两组患者术后6,12,24,48 h内血压(SBP/DBP)、心率(HR)、呼吸频率(RR)、血氧饱和度(SpO_2)的变化情况,PCIA有效/实际按压次数比值,术后VAS疼痛、镇静评分、不良反应及患者满意度。结果研究组患者术后6,12,24 h内SBP/DBP、RR、HR、SpO_2显著低于对照组,VAS疼痛评分显著低于对照组,镇静评分显著高于对照组,满意度显著高于对照组,差异有统计学意义(P<0.05)。PCIA有效/实际按压次数比值两组比较,差异无统计学意义(P>0.05)。结论右美托咪定在高血压病患者术后镇痛中镇痛效果明显,且对血压影响小,不良反应也显著减少,患者满意度高。
Objective Observe the safety and effective of dexmedetomidine on postoperative analgesia of hypertensive patients. Methods Based on the prineiple of random control and inclusion criteria, 80 cases of hypertension patients received operation in our hospital from July 2014 to July 2016 were divided into the observation group and the control group, 40 cases in each group. Patients in the control group were given appropriate intensity of Opioid through PCIA methods, while the observation group were added Dexmedetomidine on base of it. Observed and compared the blood pressure, heart rate, respiratory rate and oxygen saturation, PCIA effective/actual pressing numbers ratio, VAS postoperative pain, Ramsay score, adverse reactions of postoperative and patient satisfaction in two groups at the time points of 6, 12, 24 and 48h postoperative.Results Compared with the control group, 6h, 12h and 24h postoperative SBP/DBP, RR, HR and SpO2 of study group were decreased significantly, the VAS pain score decreased significantly as well, however, the Ramsay score and satisfaction were significantly increased ; the postoperative adverse reactions were reduced, all the differences were statistically significant（ P〈0.05 ）. The differences of PCIA effective/actual pressing numbers ratio between the two groups was not statistically significant （P〉0. 05 ）. Conclusions The effect of dexmedetomidine on postoperative analgesia of hypertensive patients is obvious and had little effect on blood pressure, in addition, the adverse reactions are significantly reduced, it got the great praise from patients.
