摘要
采用文献分析法对英国、德国、法国、比利时、荷兰等部分欧洲国家全科医生的支付方式演变历程进行梳理和分析。分析显示,各国均进行了较为全面或局部的全科医生支付方式改革,且改革具有以下特点:建立混合支付方式是对的改革方向,改革是利益集团间的博弈和互动,改革需要与分级诊疗等政策协同推进。提出建立与医保支付挂钩的混合支付的全科医生付费方式;通过建立分级诊疗、建立价格谈判机制、完善绩效评价和信息化管理平台等完善支付方式改革实施条件等政策建议。
Literature analysis was used to summarize and analyze the evolution of remuneration system of general practitioners (GPs) in UK, Germany, France, Belgium and Netherlands. The results showed that all of the countries had experienced comprehensive or incremental GPs' remuneration system reforms. There were some shared characteristics including: mixed remuneration system methods being taken as the direction of the reforms- reforms being interacted among stakeholders and being adopted along with a series'of policies such as gatekeeper system. Finally, some implications were given to remuneration system of GPs in China, which include China should introduce mixed physician remuneration methods; and establish gatekeeper system, price negotiating system, performance evaluation system and health information system to improve the reform.
