摘要
目的研究多囊卵巢综合征患者早期妊娠丢失与同型半胱氨酸(HCY)水平的相关性。方法收集2012年2月至2014年6月就诊的有早期妊娠丢失史患者及多囊卵巢综合征患者246例,其中多囊卵巢综合征流产组52例,多囊卵巢非流产组60例,非多囊卵巢综合征流产组134例。测定患者的体重、身高、HCY、空腹血糖(FBS)、空腹胰岛素(FINS)水平及性激素水平。对多囊卵巢综合征患者行3个月的去氧孕烯炔雌醇片治疗,复查HCY、FBS及FINS水平,分析HCY水平与患者早期妊娠丢失的相关性。结果多囊卵巢综合征流产组患者较其余两组患者的HCY水平明显增高,且随着流产次数的增加而增高(P<0.05);且HCY与患者HOMA-IR、睾酮水平呈正相关(r=0.553、0.503)。结论 HCY水平增高是增加多囊卵巢综合征患者早期妊娠丢失风险的重要因素,且HCY水平增高与患者的高雄激素及胰岛素抵抗有着密切关联。
Objective To study the correlation between homocysteine levels and early pregnancy loss in Polycystic ovary syndrome patients. Methods 246 patients diagnosed with early pregnancy loss or polycystic ovary syndrome were selected from February 2012 to June 2014,52 cases were divided into PCOS with abortion group,60 cases were divided into PCOS without abortion group,134 patients were divided into non-PCOS abortion group. The weight,height,HCY,FBS,FINS levels and sex hormone levels of patients were detected. The patients with polycystic ovary syndrome were given Marvelon treatment for three months,and HCY,FBS and FINS level were detected to analyze the correlation beween HCY levels and early pregnancy loss. Results The HCY level of PCOS patients was significantly higher that of the other two groups patients,and it incresed with the number of abortions（P〈0.05）. HCY levels were positively correlated with HOMA-IR,testosterone levels（r= 0.553,0.503）. Conclusion Elevated homocysteine level is an important risk factor of early pregnancy loss in patients with polycystic ovary syndrome,and elevated HCY levels in patients are closely related with hyperandrogenism and insulin resistance.
