摘要
目的:通过meta分析大黄牡丹汤联合抗生素治疗阑尾炎术后与单纯使用抗生素治疗阑尾炎术后文献的对比研究,评估大黄牡丹汤治疗阑尾炎术后的临床疗效。方法:检索建库至2016年4月中国知网(CKNI)、万方数据库、维普数据库(VIP),Pubmed、Coehrane library、Web of Science有关大黄牡丹汤联合抗生素和单纯抗生素治疗阑尾炎术后疗效的对比性研究文献。对其中的治疗的总有效率、肠鸣音恢复时间、肛门首次排气时间、切口感染、住院天数、排便时间等数据进行meta分析。数据分析使用Rev Man 5.3软件。结果:共8篇随机对照文献纳人本研究,总共745例患者纳入本次meta分析,其中阑尾切除术后使用大黄牡丹汤联合抗生素治疗的有371例患者,术后单纯使用抗生素治疗的有374例患者,大黄牡丹汤联合抗生素治疗组具有更高的治疗有效率(OR=3.65,CI:1.59~8.38,P<0.01),更快的肛门排气(WMD=-11.23,CI:-15.46^-6.99,P<0.01)和排便时间(WMD=-15.75,CI:-20.17^-11.34,P<0.01),更短的术后住院时间(WMD=-1.15,CI:-1.58^-0.72,P<0.01)以及更短肠鸣音恢复时间(WMD=-6.23,CI:-7.47^-4.98,P<0.01)和更少的术后切口感染并发症率(OR=0.29,CI:0.16~0.53,P<0.01)。结论:大黄牡丹汤联合抗生素治疗阑尾炎切除术后较单纯使用抗生素治疗有明显的疗效,值得临床推广使用。
objective：through the meta analysis of Dahuang Mudan decoction combined with antibiotics in the treatment of appendicitis postoperative literature and simple use of antibiotics in the treatment of appendicitis postoperative assessment of Dahuang Mudan decoction in the treatment of clinical curative effect after appendectomy.Methods：Searching the article up to April 2016 in Chinese knowledge network （CKNI）, Wanfang database, VIP database （VIP）, Pubmed.Coehrane library, Web of Science literature comparative study on Dahuang Mudan decoction combined with antibiotics and antibiotic therapy alone after appendectomy treatment.Total effective rate of the treatment, the recovery time of bowel sound, anal exhaust time, hospitalization time, incision infection, defecation time and other data were meta analysis data analysis using RevMan 5.3.Software.Results：A total of 8 randomized controlled trials included in this study, analysis of the meta into a total of 745 patients, including appendix resection after use the treatment of Dahuang Mudan decoction combined with antibiotics in the 371 patients, postoperative antibiotics alone there were 374 cases of Dahuang Mudan decoction combined with antibiotics in the treatment group has higher treatment efficiency （OR=3.65, CI：1.59-8.38,P＜0.01）, anal exhaust faster （WMD=-11.23, CI：-15.46 ～-6.99,P＜0.01） and defecation time （WMD=-15.75, CI：-20.17 ～-11.34,P＜0.01）, shorter hospitalization time after operation （WMD=-1.15, CI：-1.58 ～-0.72,P＜0.01） and a shorter recovery time of bowel sounds （WMD=-6.23, CI：-7.47 ～-4.98,P＜0.01） and the rate of complications of incision infection less postoperative （OR=0.29, CI：0.16-0.53,P＜0.01）.Conclusion：Dahuang Mudan decoction Combined with antibiotics in the treatment of appendicitis after resection compared with the use of antibiotics has obvious curative effect, worthy of clinical use.
