摘要
目的探讨高通量血液透析对维持性透析患者微炎症状态的影响。方法 60例维持性透析患者随机分为观察组和对照组,各30例。其中观察组给予全程高通量血液透析法治疗,对照组则给予普通血液透析治疗,治疗6个月后比较两组透析前后血清C-反应蛋白(CRP)、白细胞介素-6(IL-6)、白细胞介素-8(IL-8)、肿瘤坏死因子-α(TNF-α)、血红蛋白(Hb)、血白蛋白(Alb)和前白蛋白(PA)水平的变化。结果透析6个月后,两组患者CRP、IL-6、IL-8及TNF-α水平较本组透析前明显下降,Hb、Alb、PA水平较本组透析前明显升高,差异均具有统计学意义(P<0.05);此外透析后观察组CRP、IL-6、IL-8及TNF-α水平[(3.23±0.98)mg/L、(148.92±22.92)ng/L、(134.54±19.21)ng/L、(311.56±40.72)ng/L]明显低于对照组[(5.06±1.12)mg/L、(269.31±41.21)ng/L、(220.45±25.76)ng/L、(442.65±60.81)ng/L],Hb、Alb、PA水平[(150.95±15.70)g/L、(39.61±1.95)g/L、(313.58±50.74)mg/L]高于对照组[(128.92±14.18)g/L、(35.72±2.92)g/L、(266.60±30.51)mg/L],差异均具有统计学意义(P<0.05)。结论高通量血液透析对维持性透析患者微炎症状态具有明显的改善作用,同时效果也优于普通血液透析,值得临床推广应用。
Objective To investigate influence by high-flux hemodialysis on micro-inflammatory state in maintenance dialysis patients. Methods A total of 60 maintenance dialysis patients were randomly divided into observation group and control group, with 30 cases in each group. The observation group received whole course high-flux hemodialysis for treatment, and the control group received general hemodialysis for treatment. After 6 months of treatment, comparison was made on changes in serum C-reactive protein(CRP), interleukin-6(IL-6), interleukin-8(IL-8), tumor necrosis factor-α(TNF-α), hemoglobin(Hb), blood albumin(Alb) and prealbumin(PA) levels before and after hemodialysis between the two groups. Results After 6 months of hemodialysis, both groups had obviously lower CRP, IL-6, IL-8 and TNF-α levels than those before hemodialysis, and they had much higher Hb, Alb and PA levels than those before hemodialysis. Their differences all had statistical significance(P〈0.05). After hemodialysis, the observation group had obviously lower CRP, IL-6, IL-8 and TNF-α levels [(3.23±0.98) mg/L,(148.92±22.92) ng/L,(134.54±19.21) ng/L,(311.56±40.72) ng/L]than the control group [(5.06±1.12) mg/L,(269.31±41.21) ng/L,(220.45±25.76) ng/L,(442.65±60.81) ng/L], and its Hb, Alb and PA levels [(150.95±15.70) g/L,(39.61±1.95) g/L,(313.58±50.74) mg/L]were higher than the control group [(128.92±14.18) g/L,(35.72±2.92) g/L,(266.60±30.51) mg/L]. Conclusion High-flux hemodialysis shows remarkably improving effect for micro-inflammatory state in maintenance dialysis patients, along with better effect than general hemodialysis. This method is worth clinical promotion and application.
