摘要
目的 探讨细胞核增殖抗原(Ki-67)、角蛋白7(CK7)和角蛋白20(CK20)在膀胱癌组织的表达及预后意义.方法 应用免疫组织化学链菌素抗生物素蛋白-过氧化物酶(SP)法检测138例膀胱癌组织石蜡切片中Ki-67、CK7和CK20的表达,并结合临床随访资料进行分析.结果 Ki-67、CK7和CK20在膀胱癌组织中均高表达,阳性率分别为82.6%、88.4%和78.3%.Ki-67在膀胱癌中的表达与分级(P=0.001)、分期(P=0.007)、肿瘤复发(P=0.044)和转移(P=0.037)显著相关;CK7在膀胱癌中的表达与分级(P=0.002)、分期(P=0.043)、转移(P=0.012)、组织学分型(P=0.020)显著相关;CK20在膀胱癌中的表达与患者性别(P=0.001)、分级(P=0.012)、分期(P=0.021)、肿瘤大小(P=0.034)及肿瘤复发(P=0.035)显著相关.Spearman等级分析表明Ki-67与CK20之间(P=0.026)及CK20与CK7之间(P=0.010)呈正相关,而Ki-67与CK7之间无明显相关(P=0.319).Ki-67阳性及阴性患者的中位无进展生存期分别为10、21个月,Kaplan-Meier生存分析表明两者差异有统计学意义(P=0.000);CK7阳性及阴性患者的中位无进展生存期分别为13、9个月,Kaplan-Meier生存分析表明两者差异无统计学意义(P=0.074);CK20阳性及阴性患者的中位无进展生存期分别为9、18个月,Kaplan-Meier生存分析表明两者差异有统计学意义(P=0.000).结论 Ki-67及CK20在膀胱癌组织中的表达随着肿瘤分级分期的增高而升高,CK7的表达随肿瘤分级分期的增高而降低,三者共同参与到膀胱癌的发生发展中.
Objective To investigate the expression of proliferation cell nuclear antigen (Ki-67), cytokeratin-7 (CK7) and cytokeratin-20 (CK20) in bladder cancer and its prognostic significance.Methods The expression of Ki-67, CK20 and CK7 in 138 cases of bladder cancer tissue was detected by immunohistochemical SP method.The clinical follow-up data were analyze.Results Ki-67, CK7 and CK20 was highly expressed in bladder cancer tissues, with the positive rate of 82.6%, 88.4% and 78.3% respectively.The expression of Ki-67 in bladder cancer was significantly associated with grading (P=0.010), staging (P=0.007), recurrence (P=0.044) and metastasis (P=0.037).The expression of CK7 in bladder cancer was significantly associated with grading (P=0.002), staging (P=0.043), metastasis (P=0.012) and histological type (P=0.020).The expression of CK20 in bladder cancer was significantly associated with gender, grading, with gender (P=0.001), grading (P=0.012), staging (P=0.021), tumor size (P=0.034) and tumor recurrence (P=0.035).Spearman rank analysis showed that Ki-67 and CK20 had positive correlation (P=0.026), so as CK20 and CK7 (P=0.010).But Ki-67 and CK7 have no correlation (P=0.319).The median progression-free survival of patients positive or negative for Ki-67 was 10 and 21 months, respectively.Kaplan-Meier survival analysis revealed that the difference was statistically significant (P=0.000).The median progression-free survival of patients positive or negative for CK7 was 13 and 9 months, respectively.Kaplan-Meier survival analysis demonstrated that the difference was not statistically significant (P=0.074).The median progression-free survival of patients positive or negative for CK20 was 9 and 18 months, respectively.Kaplan-Meier survival analysis illustrated that the difference was statistically significant (P=0.000).Conclusion Along with the increase of tumor grade and stage, the expression of Ki-67 and CK20 in bladder cancer tissues was up-regulated, and CK7 down-regulated.All of them are involved in the advent and development of bladder cancer.
