摘要
目的分析医院麻醉药品不合理用药处方的原因及整改措施,规范麻醉药品的合理应用。方法运用回顾性调查方法,通过调取医院2014-2015年住院麻醉药品处方1~3级点评不合理处方607例,筛选出麻醉药品不合理用药处方236例,依据不合理用药处方分别对涉及科室、临床用途、处方类型进行分类,用帕累托图分析造成不合理用药处方的主要原因。结果统计不合理用药科室:普外科处方构成比36.44%,肿瘤内科构成比24.58%;不合理用药临床用途:麻醉辅助用药处方构成比35.17%,癌痛镇痛用药处方构成比28.81%;不合理用药处方类型:不合理用药类型中用法、用量不适宜,处方构成比48.73%,药品剂型或给药途径不适宜的和适应证不适宜的处方分别占处方构成比的19.07%、13.98%,均为主要因素。结论用帕累托图分析不合理用药处方问题的主、次因素,有针对性地对产生不合理用药因素提出整改措施,可有效提高临床合理用药水平。
Objective Analysis of unreasonable hospital narcotic drug prescriptions reasons and corrective measures to regulate the rational application of narcotic drugs. Methods Through the withdrawal of the hospital 2014 - 2015 hospital- ized narcotic drugs prescription categorized by 1 ~ 3 levels ,607 cases of unreasonable prescription were selected and 236 cases of irrational narcotic drug prescription were further screened, which were classified according to involving departments, clinical use, proscription type. The reasons for irrational use drug were analyzed by using Pareto diagram. Results Department distri- bution of irrational medical were summarized： department of general surgery constituted 36.44% comparing to composition of 24.58% in tumor internal medicine department; Irrational drug clinical use：anesthesia auxiliary proscription constituted the composition of 35.17% with the composition of 28.81% of cancer pain analgesic medication prescription;Irrational use of pre- scription：inappropriate types of medicine and dosage prescription constituted 48.73% ;inappropriate pharmaceutical dosage forms or routes of administration and unmatched prescriptions for indications accounted for 19.07%, 13.98% respectively. All of these are main factors contributed to unreasonable hospital narcotic drug prescription. Conclusion Analyzing unreasonable prescription problems of the main factors through Pareto diagrams and putting forward corrective measures by targeting unrea- sonable drug factors, can effectively improve the level of accuracy for clinical rational drug prescriptions.
