摘要
目的探讨应用3D打印技术制作的三维耳郭模型指导耳郭再造耳支架雕刻成形的价值。方法回顾性分析2014年8月至2016年12月行单侧二期耳再造术的40例患者,分为传统胶片二维耳模组(A组)和三维立体耳模组(B组),每组20例。A组按以传统的胶片二维耳模指导支架雕刻成形;B组同时以二维耳模和健侧耳的镜像三维耳模型共同指导耳支架雕刻成形。结果(1)耳支架雕刻成形时间:A组与B组手术时间比较,其差异具有统计学意义(P<0.05),且A组时间较B组时间长。(2)再造耳郭与健侧耳匹配情况:A组与B组有明显差别,B组再造耳郭形态与健侧更为相似、逼真,患者满意度更高。结论以耳郭三维模型指导耳支架雕刻成形能显著缩短手术时间,且能更好地指导个性化和精准化的耳支架雕刻成形。
Objective To investigate the value of a 3D model in guiding auricular framework carving for skin expansion auricle reconstruction.Methods Retrospective analysis was performed on 40 patients who underwent unilateral second-stage auricle reconstruction.The patients were divided into 2 groups with 20 cases per group.Two-dimensional ear model using traditional film was employed in group A,and 3D ear models were used in group B.In the process of auricular framework carving,the 2D ear model was regarded as a guideline in group A,while both 2D ear model and mirror 3D models of the contralateral ear were regarded as a guideline in group B.Results (1)The duration of auricular framework carving.There was significant difference between group A and group B (P〈0.05).The duration of auricular framework carvingin group A was longer than that in group B.(2)Match between reconstructed ear and normal ear.There was significant difference between group A and group B (P〈0.05).The appearance of the reconstructed ear was more similar to the contralateral side in group B than that in group A.The satisfaction of patients was higher in group B.Conclusion The application of 3D models in auricular framework carving of skin expansion auricle reconstruction can significantly shorten operation time.It can offer better guidance for more personal and precise auricular framework carving.
