摘要
目的评估中药联合热疗对晚期原发性肝癌的疗效及安全性。方法根据患者治疗意愿,充分考虑伦理学的前提下,采用历史对照方法,患者分为历史对照组(简称对照组)和中药联合热治疗组(简称治疗组),治疗组(21例)患者登记时间从2015年1月开始,对照组(21例)病例为近5年在我科就诊患者,选取时尽量与治疗组的基线资料相匹配。对照组仅采用补液、保肝等姑息治疗方法,治疗组采用中药联合热治疗的方法。观察两组患者疗效、生存情况及不良反应发生。结果治疗组和对照组,疾病控制率分别为52.38%和19.05%(P=0.026),经log-rank检验治疗组1年累积生存率较对照组差异有统计学意义(P=0.048),治疗组的主要不良反应为发热和血压升高,但未给予药物干预症状可自行缓解。结论中药联合热治疗对晚期肝癌患有一定疗效,无严重不良反应,而且对患者1年累积生存率有积极作用。
Objective To evaluate the therapeutic effects and safety on primary advanced liver cancer treated with Chinese herbal medicine and thermal therapy. Methods According to the patients＇ intention and under the premise of fully ethic consideration, the historical controlled method was adopted. The patients were divided into a historical controlled group （controlled group）and a combined treatment group of Chinese herbal medicine and thermal therapy（ treatment group）. The 21 cases in the treatment group were recorded since January 2015 and 21 cases in the controlled group were the outpatients in recent 5 years,which were selected in compliance with the baseline data as the treatment group. The palliative treatment such as rehydration and liver protection was used in the control group. The combined treatment of Chinese herbal medi- cine and thermal therapy was used in the treatment group. The therapeutic effects, survival situation and the adverse reactions were observed in the two groups. Results The disease control rate was 52.38% in the treatment group and was 19.05% in the control group（P =0. 026）. With the log -rank test,the accumula- tive survival rate in 1 year was different significantly between the treatment group and the controlled group（ P = 0. 048 ）. The main adverse reactions were fever and blood pressure increasing in the treatment group, which was relieved naturally without any medicinal intervention. Conclusion The combined treatment with Chinese herbal medicine and thermal therapy has a certain of therapeutic effects on advanced liver cancer, without serious adverse reactions and actively functions on the accumulative survival rate in 1 year.
