摘要
目的高血压并发不同疾病患者血尿酸水平的波动差异研究。方法选取2014年3月至2015年6月我院收治的原发性高血压患者100例为研究对象,其中单纯高血压,高血压+冠心病,高血压+糖尿病,高血压+冠心病+糖尿病依次纳入A,B,C,D四组,对比四组尿酸(SUA)、平均动脉压(MAP)水平,将四组重新分组组合,≤2种相关疾病组合为一组(A组、B组、C组),保留D组(>2种相关疾病组),比较其SUA水平,并对比高血压合并糖尿病(C+D)组与高血压未合并糖尿病(A+B)组SUA水平。结果四组UA浓度:A组>B组>C组>D组,MAP大小:A组>D组>C组>B组,差异比较均有统计学意义(P<0.05);A+B+C组SUA水平324.15±4.87μmol/L明显高于D组(P<0.05),C+D组的SUA水平333.12±1.54μmol/L与A+B组比较明显较高(P<0.05),而重新分组后的MAP比较无显著差异(P>0.05);相关分析显示SUA与其MAP呈正相关(r=+0.789,P<0.05)。结论原发性高血压患者SUA水平最高,合并疾病越多,其尿酸水平越低,糖尿病患者的血糖可能是影响合并高血压患者SUA水平的重要因素。
Objectives To study the fluctuation of serum uric acid in patients with hypertension complicated with different disea- ses. Methods From Mar. 2014 to Jun. 2015, a total of 100 patients with primary hypertension in our hospital were taken as the clini- cal research objects, and they were divided into A group (patients with simple hypertension), B group (patients with hypertension and coronary heart disease), C group ( patients with hypertension and diabetes ) and D group ( patients with hypertension, coronary heart disease and diabetes). The serum uric acid (SUA) and mean arterial pressure (MAP) levels in the four groups were compared. Then, the four groups were regrouped into A + B + C group and D group, the SUA levels of the two groups were compared, and the SUA levels of hypertension and coronary heart disease group (A + B)and hypertension and diabetes group (C + D)were also compared. Results The concentration of UA in the four groups were A 〉 B 〉 C 〉 D, the MAP levels in the four groups were A 〉 D 〉 C 〉 B ,and their differences were statistically significant (P 〈0. 05) ; the SUA level in the A + B + C group was 324. 15 ±4. 87μmol/ L which was obviously higher than that in the D group(P 〈0.05) ,the SUA level in the C + D group was 333.12± 1.54μmol/L which was obviously higher than that in the A + B group( P 〈 0. 05 ) ,there was no significant difference in MAP after regrouping( P 〉 0. 05 ) ; and correlation analysis showed that SUA was positively correlated with MAP ( r = + 0. 789, P 〈 0. 05 ). Conclusion The SUA level in patients with primary hypertension is highest; and the more complicated diseases,the lower uric acid level; and blood glucose may be an important factor affecting the SUA level in patients with hypertension and diabetes.
