摘要
目的对比经膝关节镜引导小切口与传统内固定手术治疗胫骨平台骨折的效果。方法将2014年11月~2016年8月期间的82例胫骨平台骨折患者随机分成观察组与对照组,观察组采用经膝关节镜引导小切口内固定手术治疗,对照组采用传统内固定手术治疗,比较两组的手术情况、膝关节功能恢复以及并发症发生情况。结果观察组的术中出血量、术后引流量明显少于对照组,手术时间、首次下床活动时间、骨折愈合时间以及住院时间明显短于对照组,比较差异具有显著性(P<0.05);术后2个月以及4个月后,观察组伸膝最大角度和屈膝最大角度均明显高于对照组,比较差异具有显著性(P<0.05);观察组治疗后的膝关节优良率明显高于对照组,并发症发生率明显低于对照组,比较差异具有显著性(P<0.05)。结论经膝关节镜引导小切口内固定手术治疗胫骨平台骨折手术创伤小,术后恢复快,膝关节功能恢复好,安全性高。
Objective To compare the efficacy and safety of knee arthroscopy guided mini-incision surgery and traditional internal fixation for tibial plateau fractures. Methods From November 2014 to August 2016, there were 82 patients with tibial plateau fractures randomly divided into the observation group and the control group, with 41 cases for each. The patients in the observation group were treated with knee arthroseopy guided mini-incision internal fixation, and the control group was treated by traditional internal fixation. The operation related results, knee function recovery and complications were compared between the two groups. Resnits The amount of intraoperative blood loss and postoperative drainage in the observation were significantly less than the control group, and operation time, the duration to first out of bed after operation, length of hospital stay and fracture healing time were obviously shorter than that of control group. And the difference was significant （ P 〈 0.05 ）. After 2 months and 4 months, the maximum angle of knee extension and the maximum angle of knee flexion were significantly higher in the observation group than that of the control group. And the difference was significant （ P 〈 0.05 ）. The excellent and good rate of the knee joint in the observation group was significantly higher than that of the control group, and the complication rate was significantly lower than the control group. And the differences were significant （ P 〈 0.05 ）. Conclusion Surgical treatment of tibial plateau fractures with knee arthroscopy guided mini-incision internal fixation has the advantages of less trauma, quicker recovery, better recovery of knee function and higher operative safety, which is value of more clinical promotion.
