摘要
目的探讨脑钠肽(BNP)和C反应蛋白(CRP)在急性重症胰腺炎(SAP)中的预测价值。方法我院收治的180例胰腺炎(AP)患者,根据病情轻重程度分为轻症急性胰腺炎(MAP)和SAP,选取同期健康体检者100例作为对照组。比较三组患者的生化指标及治疗后血浆BNP、CRP、左室射血分数(LVEF)水平及APACHEII评分的变化,分析CRP、LVEF及APACHEII评分与血浆BNP的相关性。结果 MAP组和SAP组的收缩压、心率、白细胞计数、甘油三酯、胆固醇、高密度脂蛋白、低密度脂蛋白、空腹血糖水平及BNP、CRP及LVEF水平均显著高于对照组,SAP组上述指标显著高于MAP组(P<0.05)。血浆BNP水平与CRP水平及APACHEII评分呈正相关,与LVEF水平呈负相关性(P<0.05)。CRP、LVEF及APACHEII评分是影响急性胰腺炎血浆BNP的独立因素。结论 BNP和CRP水平与SAP患者的病情程度有密切关系,可作为SAP的病情预测、治疗指导的参考指标。
Objective To investigate the predictive value of brain natriuretic peptide （BNP） and C reactive protein （CRP） in patients with acute severe pancreatitis. Methods One hundred and eighty patients with acute pancreatitis （AP） treated in our hospital were divided into mild acute pancreatitis （MAP） and severe acute pancreatitis （SAP） according to the severity of the disease. At the same time, 100 healthy persons were selected as control group. The biochemical indexes and plasma levels of BNP and CRP, as well as left ventricular ejection fraction （LVEF） and APACHEII score after treatment were compared among the three groups. Association of BNP with CRP, LVEF and APACHEII score was analyzed. Results The levels of SBP, HR, WBC,TG,TC, HDL-C, LDL-C and FPG as well as levels of BNP and CRP, and LVEF in the groups of MAP and SAP were all significantly higher than those in the control group.Moreover, all the indexes in the SAP group were significantly higher than those in the MAP group （-P 〈 0. 05 ）. Plasma BNP was posi- tively correlated with CRP and APACHEII score but negatively correlated with LVEF （ P 〈 0.05 ）. Further, CRP, LVEF and APACHEII scores showed to be independent factors influencing the plasma BNP in AP patients. Conclusion There is a closely relationship between the levels of BNP and CRP and the severity of SAP patients. Therefore,it can be used as a predictor of the condition of SAP and reference index for therapeutic guidance.
