摘要
目的探讨脑卒中后癫痫的临床特征。方法回顾性分析我院2013年12月至2015年10月48例脑卒中后继发性癫痫患者的临床资料。分为早发型癫痫及迟发型癫痫两组,分析癫痫与脑卒中类型、发作类型及病灶部位的关系。结果脑卒中继发癫痫的发病率为9.19%。48例脑卒中后癫痫患者中早发型癫痫19例(39.6%),迟发型癫痫29例(60.4%)。早发型癫痫中缺血性卒中8例(42.1%),出血性卒中11例(57.9%);晚发性癫痫29例中缺血性卒中25例(86.2%),出血性卒中患者4例(13.8%),二者比较差异有统计学意义(P<0.05)。早发型癫痫全面性发作5例(26.3%),部分性发作14例(73.7%);迟发型癫痫全面性发作11例(37.9%),部分性发作18例(62.1%),二者比较差异有统计学意义(P<0.05);早发型癫痫病灶位于皮层16例(84.2%),皮层下3例(15.8%);迟发型癫痫病灶位于皮层15例(51.7%),皮层下14例(48.3%)。二者比较差异有统计学意义(P<0.05)。结论脑卒中后癫痫的发生与脑卒中的类型、发作类型及病灶部位密切相关。
Objective To investigate the clinical characteristics of secondary epilepsy after stroke. Methods A retrospective analysis of 48 cases patients with secondary epilepsy after stroke from December 2013 to October 2015 in our hospital. One group of early-onset epilepsy after stroke; another group of delayed epilepsy, analyze the relationship between epilepsy and stroke type, types and lesion sites. Results The incidence of epilepsy after stroke was 9.19%. And among 48 cases of patients with epilepsy after stroke, early-onset epilepsy was 19 cases（39.6%）, delayed epilepsy was 29 cases（60.4%）. There were 8 cases with ischemic stroke among the patients of early-onset epilepsy, accounting for 42.1%, 11 cases with hemorrhagic stroke, accounting for 57.9%; 5 cases with generalized seizures, accounting for 26.3%, 14 cases with partial seizures, accounting for 73.7%; 16 cases located in brain cortex, accounting for 84.2%, 3 cases located in sub-cortex, accounting for 15.8%. There were 25 cases with ischemic stroke among the patients of delayed epilepsy, accounting for 86.2%, 4 cases with hemorrhagic stroke, accounting for 13.8%; 11 cases with generalized seizures, accounting for 37.9%, 18 cases with partial seizures, accounting for 62.1%; 15 cases located in brain cortex, accounting for 51.7%, 14 cases located in sub-cortex, accounting for 48.3%. The differences between two groups were statically significant（P〈0.05）. Conclusion The risk factors closely related to the occurrence of epilepsy after stroke are stroke type, types and lesion sites.
