摘要 目的探讨可变硬度内镜结合异丙酚TCI及BIS反馈控制在无痛肠镜中的应用.方法将56例大肠癌筛查行结肠镜检查的患者随机分为实验组和对照组,实验组采用可变硬度结肠镜结合异丙酚TCI及BIS反馈控制给药方案,对照组采用普通结肠镜结合异丙酚手控推注给药方案,观察两组患者术中和术后不良反应发生率、异丙酚总量、诱导时间、苏醒时间,比较两组麻醉效果.结果不良反应发生率实验组为32.14%,对照组为60.71%,差异具有统计学意义(P<0.05).与对照组比较,实验组异丙酚用量明显减少,苏醒时间明显缩短,两组差异具有统计学意义(P<0.05).结论采用可变硬度内镜结合异丙酚TCI及BIS反馈控制应用在肠镜中可靠安全,不良反应发生率明显减少,术后恢复迅速. Objective To study the application of variable rigidity of endoscope combined with propofol TC! and BIS feedback control in painless colonoseopy. Method 56 Patients undergoing colonoseopy for colorectal cancer screening program were randomly divided into experimental group and control group, patients in the experimental group were treated with variable rigidity colonoscopy combined with propofol TCI and BIS feedback control, and those in the control group were treated with the common colonoscopy combined with hand-controlled infusion of propofol. The intraoperative and postoperative incidence of adverse reactions, the total amount of propofol, the induction time and the recovery time were observed and compared between two groups. Results The incidence of adverse reactions in the experimental group was 32.14% and that in the control group was 60.71% ,with a significant difference (P 〈 0. 05 ). Compared with those in the control group, the dosage of propofol was significantly reduced and the recovery time was significantly shortened in the experimental group, and the difference in the results between the two groups were significant (P 〈 0.05 ). Conclusion Variable rigidity endoscopy combined with propofol TCI and BIS feedback control can be used for painless enteroscopy, which is reliable and safe. Compared with the traditional hand-control infusion, variable rigidity endoscopy combined with propofol TCI and BIS feedback control in painless enteroscopy can induce less intraoperative and postoperative adverse reactions significantly, and a faster postoperative recovery can be achieved.
