摘要
目的:探讨低糖高脂肪类肠内营养(EN)治疗在呼吸系统危重症患者的应用价值。方法:呼吸科危重症患者60例,随机分为营养组和对照组。营养组采用高脂肪低糖EN液,对照组采用普通匀浆膳。于营养治疗前1d和治疗后第14d测定血清营养相关指标,二氧化碳分压(PaCO_2)、氧分压(PaO_2)、氧合指数(PaO_2/FiO_2)、无创通气时间及使用有创机械通气的病例数、住院时间等指标。结果:治疗第14d后,营养组患者TP、ALB、Hb均较对照组显著升高(P<0.05);两组患者治疗后PaCO_2、PaO_2和PaO_2/FiO_2均较治疗前有明显改善(P<0.05);营养组患者的无创通气时间和住院时间与对照组相比有明显缩短趋势。结论:低糖高脂肪营养支持治疗能够有效的改善呼吸科危重症患者的营养状况及血气分析指标,有利于患者的恢复。
Objective:To investigate the value of sugar high fat enteral nutrition (EN) respiratory therapy in critieally ill patients.Methods:60 cases of Respiratory critically ill patients were randomly divided into nutrition group and control group (30 cases per group). Nutritional support group use high in fat and sugar EN solution, the control group using ordinary homogenized meal. Detected nutrition-related indicators in serum, carbon dioxide partial pressure (PaCO2), oxygen partial pressure (PaO2), oxygenation index (PaO2 / FiO2), the duration of NIPPV, the number of patients receiving invasive mechanical ventilation and the length of stay in ld before treatment and 14d after treatment.Results:After 14 clays of treatment, nutritional support group of patients with TP, ALB, Hb was significantly higher than the control group (P〈 0.05), PaCO2, PaO2 and PaO2 / FiO2 significantly improved compared blank groups of patients after treatment (P〈 0.05),Nutritional support patient group noninvasive ventilation and length of stay was significantly shorter compared with the control group. The use of invasive mechanical ventilation rate than the control group was significantly reduced.Conclusion:The low-sugar high-fat nutritional support can improve the indicators Respiratory critically ill patients, and promote the recovery of patients.
