摘要
目的 观察口面肌功能治疗对安氏Ⅱ1类错(牙合)畸形的早期治疗效果.方法 选取2011年1月至2014年1月,我院口腔门诊进行肌功能治疗的患者14例,年龄7~12岁,男孩3例,女孩11例.其中替牙列11例,恒牙列3例,均为安氏Ⅱ1类错(牙合),覆盖5.0~11.0 mm,其中Ⅲ度深覆(牙合)3例,均表现为牙列拥挤.进行肌功能评估后,选用不同型号的I2N训练器(Myofunctional Research Co.Queensland,Australia),进行肌功能训练.应用SPSS19.0软件对治疗前后测量数据进行配对t检验.结果 6~10个月后,患者口周肌肉功能改善,深覆(牙合)、深覆盖纠正,牙列拥挤改善,面型改善.治疗后前牙覆盖减小(4.82±1.92) mm、覆(牙合)减小(2.25±0.64) mm、上切牙唇倾度减小(2.75±2.36) mm,6.96°±6.14°差异有统计学意义(P<0.001),下前牙唇倾度增加(-2.10°±2.90°)、SNB增加(-2.04°±2.29°)、FMA增加(-1.46°±1.75°)差异有统计学意义(P<0.05).结论 口面肌功能治疗简便、无创,对安氏Ⅱ1类错(牙合)的早期治疗是有效的,肌功能评估和训练是关键,疗效依赖于患者的良好配合.
Objective To clarify the effect of orofacial myofunctional therapy on early treatment of class Ⅱ 1 malocclusion.Methods 14 patients were selected,aged from 7+3 to 12-1,with 3 boys and 10 girls.11 of them were mixed dentition and 3 were permanent dentition.The patients showed crowding,and the overjet ranged from 5.0 mm to 11.0 mm.3 of them presented with 100% overbite.All patients underwent orofacial myofunctional therapy with myofunctional appliances I2N (Myofunctional Research Co.Queensland,Australia)after myofunctional evaluation.Statistic analysis was done with SPSS19.0 software and paired t test was used.Results After 6-10 months,myofunction were improved,teeth were aligned,deep overjet and deep overbite corrected,and facial profile improved.The overjet [(4.82±1.92) mm],overbite [(2.25±0.64) mm] and proclination of the upper incisor [(2.75±2.36) mm,6.96°±6.14°] decreased after treatment and showed statistical significance (P〈0.001).The inclination of lower incisor (-2.10°±2.90°),SNB (-2.04°±2.29°) and FMA (1.46°± 1.75°) increased and showed statistical significance (P〈0.05).Conclusions Orofacial myofunctional therapy was a noninvasive and easy way to treat class Ⅱ 1 malocclusion early.Myofunctional evaluation and exercises are the key points.It is important to have patients' good compliance.
