摘要
目的:探讨噻托溴铵联合氨茶碱治疗老年慢性阻塞性肺疾病(chronic obstructive pulmonary disease,COPD)的疗效和对血清脑钠肽(BNP)、降钙素原(PCT)水平及肺功能的影响。方法:选取2014年3月—2016年8月河南省桐柏县人民医院收治的128例COPD患者作为研究对象,按随机数表法分为对照组和观察组,每组64例。在常规治疗基础上,对照组患者给予氨茶碱,观察组患者给予噻托溴铵联合氨茶碱。观察2组患者的疗效、血清BNP、PCT、肺功能改善情况及不良反应发生情况等。结果:治疗后,2组患者血清BNP、PCT水平均较治疗前明显降低,且观察组降低幅度优于对照组,差异均有统计学意义(P<0.05);2组患者肺功能均较治疗前改善,且观察组患者改善程度明显优于对照组,差异均有统计学意义(P<0.05);观察组患者的总有效率为96.88%(62/64),明显高于对照组的84.38%(54/64),差异有统计学意义(P<0.05);2组患者不良反应发生率的差异无统计学意义(P>0.05)。结论:噻托溴铵联合氨茶碱治疗老年COPD,效果较好,可有效降低血清BNP、PCT水平,改善肺功能,且安全性较高。
OBJECTIVE:To probe into the efficacy of tiotropium bromide combined with aminophylline in treatment of senile patients with chronic obstructive pulmonary disease (COPD) and effects on serum BNP, PCT level and lung function. METHODS: 128 patients with COPD admitted into Henan Tongbai County People's Hospital from Mar. 2014 to Aug. 2016 were selected to be divided into observation group and control group via the random number table, with 64 cases in each. Based on the conventional treatment, the control group were treated with aminophylline, while the observation group were given tiotropium bromide combined with aminophylline. The efficacy, serum BNP, PCT, improvement of lung function and adverse drug reactions of two groups were observed. RESULTS: After treatment, the serum BNP, PCT of two groups decreased significantly compared with before treatment, and the descend range of observation group was better than that of control group, with statistically significant difference (P 〈 0.05 ). The lung function of two groups had been improved compared with before treatment, and the improvement of observation group was better than control group, with statistically significant difference (P 〈 0. 05 ). The total effective rate of observation group was 96. 88% (62/64), significantly higher than that of control group [84. 38% (54/64) ~, with statistically significant difference (P 〈 0. 05). There was no significant difference in the incidence of adverse drug reactions between two groups (P 〉 0. 05). CONCLUSIONS: Tiotropium bromide combined with aminophylhne in treatment of senile COPD patients is significant, which can effectively decrease serum BNP, PCT level and improve lung function with high safety.
