摘要
目的观察肠内营养联合谷氨酰胺在ICU重度颅脑损伤患者中的应用效果。方法选取ICU重度颅脑损伤住院患者43例,其中男性29例,女性14例。给予肠内营养联合谷氨酰胺制剂组(研究组)23例和家庭自制饮食组(对照组)20例,分别进行营养支持,4周后观察两组患者的营养状况、血生化指标,并比较两组术后感染发生率、胃肠道并发症的差异。结果与对照组相比,研究组能更好地达到营养支持效果,其血清总蛋白、白蛋白、淋巴细胞计数明显改善(P<0.05),上臂肌围及肱三头肌皮肤厚度明显优于对照组(P<0.05),而胃肠并发症和肺部感染发生率明显减少(P<0.05)。结论肠内营养联合谷氨酰胺能有效改善ICU重度颅脑损伤患者的营养状况,降低患者并发症的发生率,为其营养支持的理想选择。
Objective To investigate the effect of combination between enteral nutrition and glutamine in patients with severe traumatic brain injury in department of ICU. Methods 43 patients (29 males and 14 females) with severe head injury in department of ICU were selected to receive the enteral nutrition glutamine preparation group (the study group, 23 patients) or the homemade diet group (the control group, 20 patients), both groups were respectively supported by nutritional. The nutritional status, the blood biochemistry, the postoperative infection and the gastrointestinal complications of both groups were observed after four weeks. Results Compared with the control group, the study group was better in the effect of nutritional support. Serum total protein, albumin and lymphocyte counts of the study group were significantly improved(P〈O.05 ), and arm muscle circumference and triceps skin thickness were significantly better(P〈O. 05), it was significantly reduced in the gastrointestinal complications and the lung infection (P〈0.05). Conclusion The nutritional status of patients with severe brain injury in department of ICU can effectively improve enteral nutrition and glutamine, the incidence of complications are reduced, the combination between enteral nutrition and glutamine is a good choice in patients with severe brain injury.
