摘要
目的了解湘潭市2010年-2016年手足口病重症患者的病原学类型及流行特征,为本地区的重症手足口病综合防控提供参考资料。方法收集2010年-2016年湘潭地区所有临床诊断为手足口病重症病例的标本,应用实时荧光RT-PCR技术检测肠道病毒通用型(EV)、肠道病毒71型(EV71)、柯萨奇病毒A组16型(Cox A16)。结果 168例手足口病重症病例中,EV阳性率为91.07%,其中EV71型病毒占70.24%,Cox A16型病毒占2.8%,其他肠道病毒占18.45%;各型别之间阳性率整体差异有统计学意义(P<0.01);发病时间主要集中在4月-6月,占76.78%,与其他月份发生率差异有统计学意义(P<0.01);不同年龄组构成比总体差异有统计学意义(P<0.01),以1岁~组最多;不同样品阳性率总体差异有统计学意义(P<0.05),以混合样品阳性率最高。结论 EV71型是湘潭地区手足口病重症病例的主要致病病毒类型,其流行具有明显的年龄、季节界限。为提高重症手足口病实验室检出率,提倡对病例多部位联合采样。
Objective To learn the etiology and epidemic characteristics of severe hand - foot - mouth disease (I-IFMD) in Xiangtan from 2010 to 2016, so as to provide reference for the comprehensive prevention and control of severe hand - foot - mouth disease in this region. Methods Specimens from patients diagnosed as severe HFMD in Xiangtan were collected from 2010 to 2016, and real -time fluorescence PCR( RT -PCR) was employed to detect general enterovirus( EV), EV71 and Cox- A16 in these specimens. Results The proportion of EV71, CoxA16 and other enteroviruses were 70. 24%, 2. 8% and 18.45% in 168 severe HFMD cases; there was statistical significance on the difference of positive rates among different types (P 〈0. 01 ) ; the high -occurrence months were from April to July, accounting for 76.78%, and compared with other months, the difference of occurrence rate was statistically significant( P 〈 0. 01 ). There was statistical significance on the difference of construction ratio among different age groups(P 〈 0.01 ), with the highest in 1 - year old group; the highest positive rate was found in the mixed samples, and the difference in the positive rate of samples was statistically significant(P 〈0.05). Conclusion EV71 was the major pathogen causing severe HFMD in Xiangtan, and the prevalence has obvious age and season limit. Multi - location sampling should be advocated to improve laboratory diagnostic rate of HFMD.
