摘要 目的探讨颈动脉超声诊断缺血性脑血管病(ICVD)的应用价值。方法选取我院收治的82例ICVD患者作为观察组,另选取同期80名健康志愿者作为对照组。对比两组研究对象的颈动脉斑块检出及分布情况、颈动脉狭窄程度与内中膜厚度(IMT)。结果观察组的颈动脉单纯IMT斑块、Ⅰ与Ⅱ类斑块检出率均明显高于对照组(P<0.01)。观察组的脂质、纤维、钙化与复合斑块检出率均明显高于对照组(P<0.01)。观察组的轻、中、重度狭窄及总狭窄率均明显高于对照组(P<0.01)。观察组的IMT为(1.04±0.21)mm,显著厚于对照组的(0.68±0.22)mm(P<0.01)。结论颈动脉超声诊断ICVD能够准确、快速地反映出患者动脉粥样硬化(AS)的病变程度与斑块类型,可作为临床检测脑血管病症的首选,值得推广。 Objective To explore the application value of carotid ultrasound in the diagnosis of ischemic cerebrovascular disease (ICVD). Methods 82 cases of patients with ICVD admitted to our hospital were selected as the observation group, and 80 healthy volunteers in the same period were selected as the control group. The detection and distribution of carotid plaques, degree of carotid artery stenosis and degree of intima-media thickness (IMT) were compared between two groups. Results The detection rates of carotid IMT pure plaques, Ⅰ and Ⅱ plaques of observation group were significantly higher than those of control group (P〈0.01). The detection rates of lipid, fibrous, calcific and compound plaques of observation group were significantly higher than those of control group (P 〈0.01). The mild, moderate and severe stenosis and total stenosis rates of observation group were significantly higher than those of control group (P 〈0.01). The IMT of observation group was (1.04 ± 0.21) mm, significantly thicker than (0.68 ± 0.22) mm of control group (P 〈0.01). Conclusions Carotid ultrasound can rapidly and accurately reflect the degree of atheroselerosis (AS) and types of plaques in the diagnosis of patients with ICVD, which can be the first choice for the detection of cerebrovascular diseases and is worthy of promotion.
