摘要
目的:测量弯针经眶-圆孔入路的相关骨性结构解剖数据,为临床安全开展此路径的三叉神经第二支疼痛射频温控热凝术提供理论参考。方法:选取我院2013年~2015年间50例颌面部良性肿瘤患者(男女各25例),拍摄头面部三维CT数据。采用Mimics软件测量眶-圆孔入路的相关解剖结构,计算穿刺针弯针弧度、骨性进针深度、圆孔外口最下点至视神经孔外口最下点距离、穿刺方向与眶耳平面角度、与矢状面角度,垂直方向上安全穿刺角度,并进行统计学分析。结果:男、女异侧组内比较,右侧∠ABA_2均高于左侧,差异具有统计学意义(P<0.05);男、女异侧组间比较,女右∠ABA_2高于男左,女右弧AC长度低于男左,男右弧AC长度、LCD、∠ABA_2、∠CAD高于女左,差异均有统计学意义(P<0.05);男、女同侧组间比较,男左、右弧AC长度、∠CAD分别高于女左、右,差异均有统计学意义(P<0.05)。其余指标比较差异均无统计学意义。结论:弯针经眶-圆孔入路避免了因直针入路方式角度的局限性,且根据Mimics软件测量数据,为临床安全开展CT引导下弯针经眶-圆孔入路提供解剖参考。
Objective:The aim of this research was to measure the anatomical data of curved needle aprroach from inferior orbital fissure to foramen rotundum approach related to the bone structure, and to provide a theoretical reference by this way for operating the radiofrequency thermocoagulation of V2 trigeminal neuralgia. Methods: Fifty patients with male and female each of 25 patients who took head and face three-dimensional CT for retrospective study. The software Mimics was used to measure the related anatomical structure of approach from inferior orbital fissure to foramen rotundum and calculated the distance from puncture point to the end point of inferior orbital fissure, distance from the end point inferior orbital fissure to the lowest exterior port of foramen rotundum, distance from the lowest exterior port of foramen rotundum to the lowest exte- rior port of the optic nerve, the radians of curved needle, depth into bone, needle angle. Then the statistical analysis was per- form. Results: The male and female of different side in the groups, right ∠ ABA2 were higher than that of the left ( P 〈 0. 05 ). The male and female of different side interblock, female right /ABA2 were higher than that of the male left, female right length of arc AC were lower than that of the male left, male right were the length of arc AC, LCD, / ABA2 and ∠. CAD were higher than that of the female left(P 〈 0.05 ). The male and female of same side interblock, male right and left length of arc AC and ∠ABA2 were higher than that of the female left and right( P 〈 0. 05 ). There were no statistically significant differences in the other indicators. Conclusion:The curved needle aprroach from inferior orbital fissure and foramen rotundum carried out under CT guidance, accordding to measurement data by the mimics software, can provided a clinical anatomical reference safely
