摘要
目的:探讨柴胡疏肝散加减对脑梗死后抑郁患者血清细胞因子影响及临床疗效。方法:选取本院神经内科诊断及治疗的脑梗死后抑郁患者84例,按随机数字表法进行分组,对照组患者42例予以舍曲林治疗,研究组患者42例在对照组基础上予以柴胡疏肝散加减治疗,采血检测血清S100β及BDNF水平、血清细胞因子水平,记录生存状况评分,同时对比临床疗效及并发症。结果:对照组治疗有效率71.43%低于研究组治疗有效率90.48%,具有统计学意义(P<0.05);与对照组比较,研究组患者治疗后血清S100β水平降低,血清BDNF水平升高,治疗后血清CRP、IL-6及TNF-α水平降低,治疗后HAMD、NIHSS及ADL评分降低,MMSE评分升高,具有统计学意义(P<0.05);两组患者不良反应多为轻中度,患者能够耐受,对症处理后缓解。结论:柴胡疏肝散加减对脑梗死后抑郁患者疗效确切,降低血清细胞因子,值得推广。
Objective ： To investigate the effect of Chaihu Shugan Powder on serum cell factors of patients with post cerebral infarction depression （PCID）. Methods：Eighty- four cases with post cerebral infarction depression （PCID） in department of neurology of our hospital were selected and according to the random number table method were divided into two groups. Forty - two cases in the control group were treated with sertraiine treatment and another cases in the experiment group received more Chaihu Shugan Powder. Serum levels of SIO0 and BDNF, serum cytokine levels, survival status score and clinical efficacy and complications were compared after the treatment. Results ：The effective rate in the control treatment group （71.43%） was lower than the experiment group＇s （90.48%） with significant difference （ P 〈 0. 05） and after treatment compared with the control group, serum S100 levels were lower and BDNF levels were higher. Levels of CRP, IL -6 and TNF -α were lower. HAMD, NIHSS and ADL scores in the study group were elevated. After treatment, the serum level was lower. HAMD, NIHSS and ADL scores were lower after treatment. MMSE scores were significantly increased with statistical significance （ P 〈 0. 05 ）. The adverse reactions of the two groups for mild to moderate were tolerant and symptomatic treatment can be recovered. Conclusion：Chaihu Shugan Powder in the treatment of patients with post cerebral infarction depression（PCID） was effective and it can reduce serum cell factors and it is worth promoting.
